About this file
This document is an affidavit form required by the Commonwealth of Kentucky for bidders, offerors, and contractors seeking to be awarded contracts or contract renewals with the Kentucky Department of Fish and Wildlife Resources (KDFWR) for a Digital Magazine Subscription & Payment Site project. The affidavit serves as a sworn attestation under penalty of perjury and remains effective for one year from the date of execution. The form requires completion and notarization by an authorized representative of the contracting party before submission to the Commonwealth. The document outlines three distinct affidavit sections: a campaign finance compliance attestation confirming that the contractor has not knowingly violated Kentucky campaign finance laws, a resident bidder status affidavit requiring verification that the entity is authorized to transact business in Kentucky and has maintained corporate income tax filings, unemployment insurance fund payments, and workers' compensation coverage for one year prior to and through the advertisement date, and a qualified bidder status affidavit for entities claiming nonprofit or disabled worker employment status under Kentucky Revised Statutes Chapter 163 and 200 KAR 5:410(3).
The Commonwealth reserves the right to request updated affidavits at any time and to demand documentation supporting claims of resident bidder or qualified bidder status. Failure to provide requested supporting documentation upon demand will result in bidder disqualification or contract termination. Each affidavit section requires the signature, printed name, title, and date of an authorized representative, along with notarization before a notary public. The form includes spaces for bidder or offeror name, address, Kentucky vendor code if known, state and county of notarization, and notary commission expiration date. No specific pricing terms, contract value, set-asides, award dates, or contract term information is contained within this affidavit document.
