Attention: Mr. Rick Rogers & Ms. Crystall Wells

Subject: Building 1238, Pre-Renovation Asbestos Survey

Florida State Hospital, Chattahoochee, Florida

Dear Mr. Rogers:

As requested, Southern Earth Sciences, Inc. has completed an asbestos survey of Building 1238 at the Florida State Hospital in Chattahoochee, Florida. The survey did not include the occupied (laboratory) area of the building and roofing samples were not collected to prevent damaging the existing roofing. We understand that the building may be renovated.

1.0 INTRODUCTION

On April 18, 2023, asbestos surveyors with our firm obtained sixty (60) bulk samples of suspect asbestos-containing building material for analysis. Eighty-eight (88) samples were analyzed after separation of layered materials. The samples consisted of pipe insulation (white chalk-like, fiberglass with mudded elbows, fiberglass with black mastic/jacket, foam glass insulation with mastic), HVAC duct mastic, floor tile (9”x9” and 12”x12”, various colors), floor tile mastic, ceiling tiles, plaster, and window glazing. The bulk samples were sent to Eurofins CEI, an analytical laboratory in Cary, NC. Bulk samples were analyzed by Polarized Light Microscopy (PLM), E.P.A. Method 600/R-93/116. Test results and sample locations are included in the attached Asbestos Analytical Report / Chain of Custody.

2.0 DEFINITIONS

Asbestos Containing Building Materials (ACBM): Building materials used for construction of a structure that are known or are suspect for containing asbestos.

Asbestos: Asbestos is the asbestiform varieties of chrysotile, crocidolite, amosite, anthophyllite, tremolite, and actinolite.

Florida State Hospital Building 1238

Asbestos Inspection: An evaluation performed by a trained and E.P.A. certified inspector to determine the presence or absence of Asbestos-containing materials. Asbestos inspectors engage in the survey and assessment of ACBM.

Category I non-friable ACM: asbestos-containing packing’s, gaskets, resilient floor covering and asphalt products.

Category II non-friable ACM: any material, excluding Category I ACM, that when dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure.

Demolition: the removal of load-bearing walls or structural components.

Environmental Protection Agency (EPA)

Regulated Asbestos Containing Material (RACM): (a) Friable asbestos materials, (b) Category I non-friable ACM that has become friable, (c) Category I non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading, or, (d) Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition or renovation operations regulated by NESHAPS.

Renovation: the removal of any other building components other than load-bearing walls or structural components.

3.0 PHYSICAL SURVEY

The building is a 2-story building with a basement and crawl space. The first-floor east wing of the building contained the laboratory which was not included in our survey. Floor plans showing areas not accessible are attached.

Exterior window glazing was located on the windows. Screens covering window openings were in place and we could not determine if caulking was present or sample that material (if present).

A crawlspace was located under the south section of the building. We did not have access to the crawlspace under the laboratory (if present). The floor in the crawlspace was exposed soil.

Pipe insulation (white chalk-like) and elbow insulation debris was observed in the crawlspace. Intact pipe insulation included fiberglass piping with paper jacket with mudded elbows and white chalk like insulation.

The air handling room/mechanical room had fiberglass piping with paper jacket and mudded elbows and foam glass pipe insulation with black mastic. Duct insulation was fiberglass with black duct mastic.

The elevator lobby and hallway had white chalk-like pipe insulation, fiberglass pipe insulation with mudded elbows, and fiberglass insulation with pvc/plastic elbows. The lower mechanical space had fiberglass insulation with pvc/plastic elbows. Over the high voltage room was fiberglass piping with mudded elbows which continued to the north caged storage room.

Upper floors – The ceilings were suspended ceiling tiles under fiberglass suspended ceiling tiles under concrete deck. Ceilings in the bathrooms, janitors’ closets and work rooms were plaster.

The lower portions of all walls were covered with ceramic tile. Walls were plaster in the hallways, bathrooms, storage rooms and patient rooms. The floors were 9”x9” vinyl floor tile or 12”x12” vinyl floor tile. There were two layers of vinyl floor tile in the nurse’s station area.

Ceramic tile was located on the floors in the bathrooms.

Pipe insulation (fiberglass with mudded elbows and fiberglass with black mastic) was located in pipe chases and above plaster ceilings in the bathrooms. Enclosed in pipe chases in the corners of patient rooms was piping with fiberglass insulation. Piping associated with the radiators was metal covered with neoprene. No mastic was observed on the radiator piping. Duct mastic was associated with individual air conditioning units in patient rooms. Duct mastic was also present above suspended ceilings in the hallways and in the ceiling space in the nurse’s station area.

Stairwells had concrete stairs, plaster walls and ceilings.

4.0 SUMMARY OF FINDINGS

The E.P.A. definition for an asbestos-containing material is a building material that contains more than 1 percent asbestos when analyzed by PLM and is placed into two categories: friable and non-friable. Friable ACM is a material that can be easily pulverized with hand pressure as opposed to non-friable ACM.

4.1 FRIABLE ACM

The white chalk-like pipe insulation contains 15% Chrysotile asbestos. The pipe insulation was located in the crawlspace and in the hallway in the basement. Debris was noted in the crawl space. There was approximately 250 linear feet of the white chalk-like pipe insulation.

Mudded elbow insulation contained 2-15% Chrysotile asbestos. The elbows are associated with fiberglass pipe insulation. Elbows were noted in the crawlspace, basement hallway, basement caged area, above plaster ceilings in bathrooms on upper floors and in pipe chases in bathrooms and patient rooms. There was approximately 1300 linear feet of pipe insulation with elbows in the basement. We have ESTIMATED approximately 460 linear feet of pipe insulation in patient room pipe chases, 700 linear feet of pipe insulation above plaster ceilings in bathrooms, in bathroom pipe chase and in pipe chases in the nurses’ station areas. We should note pipe insulation debris (ACM) was observed in the lower level of the mechanical room near the high voltage room and in the crawlspace.

4.2 NON-FRIABLE ACM

Black mastic on foam glass pipe insulation contained 5% Chrysotile asbestos. The foam glass pipe insulation was located in the mechanical room/HVAC room in the basement. There was approximately 75 linear feet of foam glass insulation with ACM black mastic.

The black mastic on pipe insulation contained 7% Chrysotile asbestos. The pipes with the mastic were observed in pipe chases in the bathrooms. There was approximately 1100 linear feet of this insulation in pipe chases.

The mastic on the HVAC ductwork in the basement contained 5% Chrysotile asbestos. There was approximately 150 linear feet of duct work in the basement. There was approximately 4000 linear feet of duct mastic in the upper floors.

The 9”x9” vinyl floor tile (various colors) contained 2-3% Chrysotile asbestos. The floor tile was located in patient rooms, hallways and storage rooms. The 9”x9” floor tile was also located under 12”x12” vinyl floor tile in the nurse’s station area. There was approximately 22,000 square feet of floor tile. We should note that none of the floor tile mastic samples tested positive for asbestos.

5.0 CONCLUSIONS AND RECOMMENDATIONS

In accordance with the National Emission Standards for Hazardous Air Pollutants (NESHAPS), 40 CFR Part 61, Subpart M, Regulated Asbestos Containing Materials (RACM) are required to be removed prior to renovations that would disturb asbestos containing materials and prior to demolition.

At this time, we do not know what renovations are planned for the building. Any plans to remove flooring or update HVAC system and plumbing would require removal of asbestos containing materials.

Please be advised that there was friable asbestos containing debris observed in the crawlspace and in the lower level of the mechanical room near the high voltage room. The asbestos debris must either be removed by a Florida licensed asbestos abatement contractor or restrict access to these areas to persons with proper asbestos training.

Removal of asbestos containing materials must be performed by an Asbestos Abatement Contractor licensed by the State of Florida, with certified personnel. ACM abatement must comply with the State of Florida Department of Business and Professional Regulation, Chapter 469, licensing, and training; Chapter 62-701.520, Waste Disposal Rules; Chapter 62-257, DEP 1999, Asbestos Program and OSHA 29 CFR 1926.1100 (Construction Industry Standard).

NESHAPS requires a 10-working day notification to the Florida Department of Environmental Protection (FDEP) Division of Air Management prior to the start date of an asbestos abatement project and/or prior to demolition.

6.0 GENERAL COMMENTS

This asbestos survey has been performed to identify asbestos containing materials in the existing buildings and is not intended as abatement specifications and drawings. Quantities of materials would be verified during preparations of plans and specifications. We should note that destructive sampling was not performed, and additional materials may be located within wall chases and above plaster ceilings.

Comments and observations given above reflect an opinion as to the various materials and conditions visually observed during the inspections and should not be construed as a representation or warranty expressed or implied, as to scope, thoroughness, or accuracy of the inspection.

A conscious effort is made to identify all suspect materials. There is a possibility that conditions or materials may exist which could not be identified during our survey due to physical inaccessibility and the use of nondestructive sampling methods. Materials that typically do not contain asbestos have not been sampled. These materials include but are not limited to plastics, wood, fiberglass, etc.

Conclusions and recommendations given in this report are based upon our interpretation of current regulatory standards. Changes in regulatory standards may require changes in our conclusions and recommendations.

We appreciate the opportunity to be of service to you on this project. Should you have any questions or require additional information, please contact our office.

Sincerely, SOUTHERN EARTH SCIENCES, INC.

Due to the limitations of the EPA 600 method, nonfriable organically bound materials (NOBs) such as vinyl floor tiles can be difficult to analyze via polarized light microscopy (PLM). EPA recommends that all NOBs analyzed by PLM, and found not to contain asbestos, be further analyzed by Transmission Electron Microscopy (TEM). Please note that PLM analysis of dust and soil samples for asbestos is not covered under NVLAP accreditation. Estimated measurement of uncertainty is available on request.

This report relates only to the samples tested or analyzed and may not be reproduced, except in full, without written approval by Eurofins CEI. Eurofins CEI makes no warranty representation regarding the accuracy of client submitted information in preparing and presenting analytical results.

Interpretation of the analytical results is the sole responsibility of the client. Samples were received in acceptable condition unless otherwise noted. This report may not be used by the client to claim product endorsement by NVLAP or any other agency of the U.S. Government.

Information provided by customer includes customer sample ID and sample description.
