About this file
This is an Insurance Requirements Schedule issued by the State of Nevada Risk Management Division (2nd Edition, effective May 9, 2016) for standard professional service contracts, including the phlebotomy services solicitation for DUI arrests in Pershing County, Nevada. The document establishes mandatory insurance coverage requirements that contractors and subcontractors must procure and maintain throughout the duration of contract performance and any applicable warranty periods.
Contractors are required to maintain Commercial General Liability coverage with a general aggregate limit of $2,000,000 and each occurrence limit of $1,000,000; Automobile Liability with a combined single limit of $1,000,000 for owned, hired, and non-owned vehicles; Workers' Compensation and Employers' Liability with statutory limits including $100,000 per accident and $500,000 disease policy limit; and Professional Liability (Errors and Omissions) with limits of $1,000,000 per claim and $2,000,000 annual aggregate. The State of Nevada must be named as an additional insured on all applicable policies, and all coverage must be primary and non-contributory. Contractors must provide certificates of insurance (ACORD form or equivalent) prior to work commencement and maintain continuous coverage throughout the project. All insurers must be licensed in Nevada with an A.M. Best rating of at least A-VII. Cancellation of any required policy requires thirty days' prior written notice to the State (ten days for non-payment of premium), and failure to maintain required insurance constitutes material breach of contract. Subcontractors must be included as additional insureds under the contractor's policies or provide separate certificates, with all coverages subject to the same minimum requirements.
