About this file
This is an Insurance Requirements Schedule document from the State of Nevada's Risk Management Division, applicable to professional service contracts, including the phlebotomy services solicitation for the Highway Patrol in the Gardnerville-Minden area. The document establishes mandatory insurance coverage requirements that the contractor and all subcontractors must maintain throughout the contract term and any warranty periods. The contractor must obtain Commercial General Liability with a general aggregate limit of $2,000,000 and per-occurrence limit of $1,000,000; Automobile Liability with a combined single limit of $1,000,000 for owned, hired, and non-owned vehicles; Worker's Compensation and Employers' Liability with statutory coverage and specific accident and disease limits; and Professional Liability (Errors and Omissions) with per-claim and annual aggregate limits of $1,000,000 and $2,000,000 respectively. All policies must name the State of Nevada as an additional insured, include waiver of subrogation provisions where applicable, and maintain primary and non-contributory status relative to other insurance sources.
The contractor is required to provide certificates of insurance on ACORD forms or state-approved equivalents prior to work commencement, with all policies effective at or before the start date and maintained for the contract duration. Insurance must be placed with carriers rated A-VII or better by A.M. Best and licensed to operate in Nevada. The contractor must provide thirty days' notice of cancellation or non-renewal (ten days for non-payment of premium) to the designated state representative, and failure to maintain required coverage constitutes material breach. The state reserves the right to request complete certified copies of all policies and may approve modifications to insurance requirements through the Risk Management Division or Attorney General's Office. All subcontractors must be included as additional insureds under the contractor's policies or provide separate certificates meeting the same minimum requirements.
