About this file

This is a pricing schedule template for a Request for Quotations (RFQ) issued by the Indian Health Service (IHS), Phoenix Area Indian Health Service (PAIHS), for nonpersonal primary care physician services. The solicitation (RFQ-26-PHX-052) seeks to establish one or more Blanket Purchase Agreements (BPAs) for Family Medicine and Internal Medicine physician services supporting government-operated healthcare facilities throughout the Phoenix Area. This acquisition is set aside exclusively for Indian Small Business Economic Enterprises (ISBEEs) under the Buy Indian Act and is being conducted using FAR Part 12 commercial services procedures.

The pricing schedule attachment is a template document (Attachment B - Pricing Schedule.xlsx) provided for vendors to complete and submit with their quotations. The file itself does not contain specific pricing information, response deadlines, award dates, or other transactional details; rather, it is a blank form that establishes the format and structure vendors must use when submitting their pricing for physician services. Vendors responding to RFQ-26-PHX-052 are required to utilize this template to detail their rates and pricing terms for the requested Family Medicine and Internal Medicine physician services that would be ordered through individual BPA calls during the contract ordering period.

The Government may issue BPA Calls for one or more Ordering Activities/CLINs identified in this Pricing Schedule.

The Government reserves the right to issue BPA Calls for any combination of Ordering Activities/CLINs identified in this Pricing Schedule. Inclusion of an Ordering Activity in this Pricing Schedule does not guarantee issuance of a BPA Call.

Instructions:

The Government will evaluate pricing using the estimated annual quantities identified below. The estimated quantities are provided solely for quotation evaluation, acquisition planning, and contract administration purposes. They do not establish a guaranteed minimum quantity of services.

Quoters shall provide one fully burdened hourly rate for each CLIN identified in this Pricing Schedule. Hourly rates shall include all direct and indirect costs associated with contract performance as described in Section B of the solicitation.

The fully burdened hourly rates shall constitute full compensation for all authorized hours worked, including services performed during day, night, weekend, and Federal holiday shifts. Separate overtime, holiday, callback, standby, on-call, shift differential, or other premium rates shall not be proposed. The Government will not separately reimburse travel, lodging, meals, overtime, holiday pay, shift differential, administrative costs, recruiting costs, credentialing costs, or any other direct or indirect costs not expressly identified in the awarded hourly rate.

Hourly rates and evaluated prices shall be expressed in U.S. dollars and rounded to two (2) decimal places.

Quoters may submit a supplemental pricing narrative or pricing worksheet in accordance with Section D.6 of the solicitation. In the event of any inconsistency, the Government-issued Pricing Schedule shall govern for purposes of quotation evaluation and award.

The Government may verify mathematical accuracy and correct apparent clerical or arithmetic errors for evaluation purposes.

Hourly rates shall remain fixed for the BPA ordering period unless adjusted in accordance with the terms and conditions of the BPA.
