About this file
This is a Past Performance Questionnaire template for use in evaluating contractors' historical performance under Request for Quote (RFQ) No. 36C10X27R0002 for VA Loan Servicing. The questionnaire is administered by the Department of Veterans Affairs Strategic Acquisition Center Frederick and is distributed to references provided by offerors to assess contractor qualifications for award consideration.
The evaluation form requires references to rate contractors across six standard performance areas using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) plus a Not Applicable option: Quality of service delivery against contract performance parameters; Schedule adherence including timeliness of milestone completion and administrative requirements; Cost Control (applicable only to cost-type contracts); Management effectiveness including coordination, problem identification, corrective action, and customer satisfaction; Utilization of Small Business compliance; and Regulatory Compliance including reporting, subcontractor payment, and suspension/debarment status. The template includes three additional customizable evaluation areas and requires detailed comments (up to 24,000 characters per section) to support each rating. Completed questionnaires must be submitted to three VA evaluators: Roxana.Cepeda@va.gov, Won.Chae@va.gov, and Shawn.Johnson6@va.gov. The form concludes with an overall recommendation section asking whether the reference would award the contract to the contractor based on their demonstrated ability to execute their proposal.
