About this file
This is a Past/Present Performance Questionnaire form for evaluating contractor performance on solicitation number 36C24426Q0704 for the Butler Veterans Affairs Medical Center in Pennsylvania. The form is designed to be completed by government evaluators (such as Contracting Officers, Program Managers, or Technical Representatives) to assess past or current contractor performance on related contracts.
The questionnaire requires evaluators to provide detailed narratives and adjectival ratings across three primary assessment elements: Quality of Services/Care (addressing the contractor's ability to provide therapeutic and rehabilitative services in a safe environment to homeless eligible Veterans, including meals, transportation, laundry facilities, gender-specific and ADA accommodations); Qualified Personnel (evaluating the contractor's ability to provide experienced managers and supervisors); and Business Relations (assessing the contractor's willingness to correct compliance issues, problem resolution timeliness, initiative, flexibility, and working relationship effectiveness). Evaluators must also identify overall contractor strengths and weaknesses and provide an overall past/present performance rating using a five-point adjectival scale: Excellent, Good, Satisfactory, Marginal, or Unsatisfactory. Completed forms should be submitted to Enrico.Tan1@VA.GOV with the solicitation number and contractor name referenced in the subject line. The form includes space for contractor identification information, evaluator details, and a final recommendation on whether the government would award to the contractor again.
