About this file

This document is a comprehensive Community Health Profile for the Fallon Health Center, part of the Phoenix Area Indian Health Service (IHS). The profile provides a detailed assessment of the current healthcare facility, its service area, population demographics, and strategic planning for 2035. Key highlights include:

The Fallon Tribal Health Center serves the Fallon Paiute-Shoshone Tribe and surrounding communities in northwestern Nevada, with a current user population of 1,411 expected to grow to 2,373 by 2035. The facility currently operates across 8 buildings totaling 31,504 square feet on 4.4 acres, with significant infrastructure challenges including aging equipment (mostly from 1997), inadequate space for patient volume, and multiple modular buildings. The health center's mission is to provide comprehensive, culturally sensitive healthcare to American Indian populations, offering services including primary care, dental, behavioral health, optometry, and specialty care. The strategic plan emphasizes expanding services, improving facility infrastructure, and addressing critical needs such as specialty care access and facility consolidation.

The Fallon Paiute‐Shoshone Tribe is a federally recognized Native American

Tribe located in the northwestern part of the State of Nevada – the Tribe is affiliated with the Toi‐Ticutta (cattail eaters) band of the Northern Paiute and Western Shoshone people. The Tribe is a non‐IRA Tribe established through executive order and is governed by the Fallon Business Council. The

Tribe has a resident population estimated at 1,500 individuals with about

88% being Native Americans. The Tribe is comprised primarily of two communities – the Stillwater Reservation, about 8,500 acres and is located about 12 miles from the city of Fallon, and the Fallon Colony, about 60 acres and is located about one mile from the city of Fallon.

The Tribe started the Fallon Tribal Health Center (FTHC) in the late 1980’s through a 93‐638 contract and over the years as taken all its shares except for the PRC program (which is currently planned for CY2022‐2023). The

FTHC serves three tribal communities – the Fallon Tribe, as well as the

Lovelock Paiute Tribe and Yomba Shoshone Tribe. This was done by resolution of the other two Tribes to allow the Fallon Tribe to handle their shares since these two Tribes are too small to operate their own clinics and the FTHC is the closest tribal health facility to these two Tribes. The FTHC offers medical care (family health, podiatry, chiropractic, audiology), laboratory services, optical and ophthalmology, dental and endodontic, behavioral health (mental health and substance abuse), pharmacy, diabetes program, CHR Program, a PRC Program (to coordinate with IHS), a wellness/fitness center and an administrative/business side (patient registration, environmental services, medical records, billing, CAC, etc.)

Fallon Health Center is located in the town of Fallon, NV, approximately 63 miles east of Reno, NV and Carson City, NV. The Health Center is located on it's Colony which is 10‐15 miles from the main population center on the

Reservation.

The Fallon Tribal Health Center (FTHC) is a tribally owned and operated clinic, located on the Fallon Paiute Shoshone Indian Colony at 1001 Rio Vista Drive, Fallon, NV. The health center is committed to enhancing the quality of life of all of American Indians by providing a culturally competent and patient‐ centered continuum of care.

It is a hub of primary care for Native Americans in the Churchill County area.

The Health Center also provides primary care to Lovelock and the Yomba

Tribe which are located 60‐110 miles away (one‐way). Fallon sends most of its referral workload to Reno, NV.

The Fallon Service Area desires to realize the following health outcomes in 2035.

• Recognition of the cost differences in regions of the

Phoenix Area Office and proper allocation of funds to cover such differences.

• Appropriate funding to the Tribes through their 93‐638 contracts/compacts to provide quality healthcare to the

Native American population in our service area.

• Establishments of a program by IHS to provide for upgrades/updates to medical, dental, and optical equipment.

• Justification of need and funding allocations to provide for specialty and urgent care services in areas where Phoenix

Indian Medical Center is not a viable option such as in northern Nevada.

• Justification for replacement of the current facility to allow the FTHC to operate in a single building instead of having one main facility and four (4) modular buildings.

• Establish more satellite services into the remote areas served by the Health Center to provide more preventive healthcare and family provider services instead of working on urgent matters in which a patients has devolved to providing critical and stabilizing care.

Development of a Resource Tool that can better help the FTHC search out funding for facility expansion and introducing needed services.

A guide for the Indian Health Services to better coordinate with Tribes in the northern Nevada Area to address the health shortfalls and disparities that we are facing.

Identification of much needed hospital and/or urgent care facility in the northern Nevada area to fill a critical need since Phoenix Indian Medical

Center is too far from the Tribes in this area to access for most type of services.

Identification of health care services, as well as the supporting services, that are needed for our population in the short and long‐term basis, including any remote/mobile services.

Identification of current, actual, and needed space to provide quality healthcare services in the short and long‐term basis.

A plan to help the FTHC and FPST better layout services to meet the needs of our three tribal communities and that aligns with our goals and objectives.

The Fallon Tribe does not have an operational transit program currently though it is in development. The only method of patients to get to their appointments is from CHR Program. The Fallon Tribal Health Center has a

CHR Program with three full‐time personnel (one supervisor who drives as well and two drivers) for the Fallon area.

Transportation

Patients face multiple challenges in accessing referral care. Cost and lack of access create difficulties for patients traveling for care. Referral service most impacted include imaging, emergency room visits, urgent care services, specialty care, inpatient hospitalization and outpatient surgery. Patients in need of some basic and specialty services, such as timely mental health counseling, have no where else to go, and for service such as substance abuse residential care, are often referred out of state. Sending referrals to PIMC is a direct expense to Fallon as most patients lack the financial resources to travel to Phoenix. Instead, Fallon will use their own PRC funds for local providers in

Reno or avoid treatment altogether. Finding providers in Reno who will accept IHS funding is limited due to their inability to be paid in a timely manner. Collaboration with referral sources is limited including the sharing of medical records and the coordination of care with social services.

Fallon Health Center is planning on expanding ambulatory, ancillary and specialty services to limit the need to refer patients out. They are also planning on increasing tele‐medicine and housing more visiting providers from the community to decrease the need for referrals.

Service Area Referral Plan

Fallon Health Center operates as a 40 hour per week

Health Center with complete primary care, dental, behavioral health, optometry, podiatry, audiology and supporting ancillary services. New services anticipated for 2035 include; rehab services, imaging services, additional preventive care, and additional specialists. There is no extended service area planned, although it is anticipated that Lovelock, Winnemucca, and Yomba will send various workloads to Fallon Health Center.
