About this file

This document is an Indian Health Service (IHS) Buy Indian Act Indian Economic Enterprise (IEE) Representation Form. The form requires offerors to self-certify their status as an Indian Economic Enterprise when responding to a solicitation, sources sought notice, or request for information. Key requirements include maintaining IEE status at the time of offer, contract award, and throughout the entire contract performance period. Offerors must be 51% owned by members of a federally recognized tribal entity and must immediately notify the Contracting Officer if they no longer meet eligibility requirements. Successful offerors must also be registered in the System of Award Management (SAM). False or misleading information can result in legal penalties under 18 U.S.C. 1001 and other federal statutes. The form provides space for the offeror to indicate whether they meet the IEE definition and requires details such as the name of the 51% owner, tribal entity, business name, and UEI SAM number.
