BASIS OF AWARD
• Evaluations shall be made utilizing "Best Value”- The offer determined to be the most advantageous to the Government.
• Contractor must provide documentation showing ability to provide service(s) listed in accordance with specifications listed in the statement of work document.
• Contractor must provide evidence of insurance with minimum amount of $1,000,000.00.
• Compliance with insurance provisions “General Liability”, “Workman’s Compensation”, and
“Evidence is required.
• Delivery capability by the identified required date
• Bidder must be registered at Sam.gov.
Acceptable
ALL of the minimum acceptable criteria are met by the proposal. The offeror's proposal meets the performance and technical capability requirements defined in the Statement of Work.
NOTE: Once the proposals have been determined to be "technically acceptable," the award will be based on cost/price only.
Unacceptable
Not all of the minimum acceptable criteria are met by the proposal. An unacceptable proposal contains one or more deficiencies. The proposal fails to meet specified minimum performance and technical capability requirements defined in the SOW. Offer’s not deemed technically acceptable will be removed from further consideration of the contract award.

Evaluation Criteria:
1. Contractor must provide documentation showing ability to provide service(s) listed in accordance with specifications listed in the statement of work document. Documentation to include:
a. Copy of Hazardous Material CDL for each employee who will be driving transport trucks
b. Proof of employee training including Bloodborne Pathogen Training and Waste
Handling within last year.
c. List of and Location of all TSD Facilities used and any subcontractors
d. Contingency Plan for Emergency Response and Clean up.
2. Contractor must provide evidence of insurance with minimum amount of $1,000,000.00 for the transport of regulated medical waste, to include evidence of compliance with insurance provisions regarding “General Liability”, “Workman’s Compensation” and “Evidence of
Coverage”.
3. Delivery capability by the identified required date as well as contingency plan
4. Bidder must be registered at Sam.gov.
5. Contractor shall demonstrate that all drivers possess a current hazardous material CDL and have completed a training course on bloodborne pathogens and a training course on waste handling within the last year prior to start of contract.
6. Contractor must supply containers that meet, at a minimum, the following salient characteristics:
1. Reusable
2. Puncture Proof
3. Must arrive at the medical center fully assembled
4. Leak proof in all orientations
5. One hand/one step disposal mechanism
6. Overfill protection design feature built into container (i.e., container must close off automatically once fill point has been reached)
7. Secure/Tamper-Proof to include temporary and permanent locking design features
8. Sight/View port that shows when the container is full
9. 12” Large access Horizontal Drop opening with safety tray restricting hand access for public access areas and open top unrestricted access for secure areas such as lab and OR
10. Available in various sizes dependent upon need (2, 4, 8 & 16 Gallon)
11. Must have a barcode tracking feature on each container for safety and accountability
13. Able to be sanitized using a six-stage wash process to include environmentally friendly cleaning agents to ensure a high level of microbiological efficacy
14. Must meet U.S. Department of Transportation regulations
15. Must be certified to United Nations PG2 specifications for transport of dangerous goods
16. Must be labeled in accordance with Federal, State, Local regulations
All containers and/or outer cabinets shall be clearly labeled with the biohazard symbol and the word “biohazard” as required by local, state and federal regulations.
Management Quality Control
• A description/schedule of the offeror’s plan to provide collection and disposal of regulated medical waste from all VA facilities listed in paragraph 1.3 of SOW, as detailed in the
Statement of Work (SOW). This could be as simple as a staffing plan/weekly schedule that identifies the personnel who will be providing the services, where they will be providing the services, and when they will be providing these services (days of the week).
Past Performance:
Contractor must have (3) years’ experience servicing similar hospitals with sharps containers.
Contractor shall provide (3) references f rom previous hospitals where similar work was performed.
