About this file
This is the NAVFAC/USACE Past Performance Questionnaire (Form PPQ-0), a template used to evaluate contractor performance on federal contracts. The form is designed to be completed collaboratively, with contractors providing contract and project details in Blocks 1-4, while clients or contracting officers complete Blocks 5-8 with their assessment and evaluation.
The questionnaire evaluates contractor performance across seven primary categories: Quality (technical data preparation, quality standards compliance, problem resolution, and quality control programs); Schedule/Timeliness of Performance (delivery schedule compliance and resource utilization); Customer Satisfaction (end-user satisfaction, contractor cooperation, and overall satisfaction); Management/Personnel/Labor (on-site management effectiveness, workforce qualification, government property control, personnel expertise, small business utilization, multi-project management, requirement changes, and overall management effectiveness); Cost/Financial Management (cost compliance, innovative cost reduction methods, invoice timeliness and accuracy, accounting system adequacy, contract terminations, and financial stability); Safety/Security (safety plan adherence and security requirement compliance); and General (emergency response capability, contractual compliance, and overall work performance). Each performance element uses a standardized five-point rating scale: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Not Applicable (N). The form includes detailed definitions for each rating level and requires clients to submit completed questionnaires directly to offerors, who then incorporate them into proposals submitted to USACE. The government reserves the right to verify all information provided.

The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.
