About this file
This is a bid sheet attachment for a Request for Bids (RFB 758 2600000672) issued by the Commonwealth of Kentucky for off-site data and document storage services. The procurement seeks vendors to provide comprehensive storage solutions including pickup and delivery services across three locations: Franklin County, Jefferson County, and Boone County (alternate data center location). Services must be available during standard work weeks (Monday-Friday), weekends (Saturday-Sunday), and emergency/rush periods operating 24/7 at agency specifications. The bid sheet requires vendors to propose pricing for both media item storage and closed container storage, with differentiation between temporary data backup storage (frequent retrieval, active storage) and archival data/document storage (infrequent retrieval). The current facility maintains approximately 80 cases in storage. Courier service rates must be all-inclusive of librarian filing fees, retrieval and access costs, and round-trip transportation regardless of delivery/pickup volume.
Pricing evaluation will account for 80 maximum points, distributed across pickup/delivery services (20 points for work week, 10 points for weekend, and 10 points for emergency services per location) and monthly storage fees (10 points each for temporary and archival storage by media type and container type). Years in service comprises 20 maximum points; vendors must demonstrate a minimum five-year experience requirement with points awarded only for experience exceeding this threshold up to a maximum of 20 years. Bidders failing to meet the five-year minimum experience requirement will be deemed non-responsive. Vendors are instructed not to revise the bid sheet format, as any modifications will result in non-responsive bids. Archival storage services exclude requirements for 24-hour, 365-day access, vendor inventory management, and rush delivery capabilities.
