About this file
This file is an affidavit document issued by the Commonwealth of Kentucky for bidders, offerors, and contractors seeking to be awarded or renew contracts related to guest room amenities. The document contains multiple required affidavits that must be completed, notarized, and submitted by authorized representatives of contracting parties. The primary affidavit requires attestation under penalty of perjury that the contractor has not knowingly violated Kentucky campaign finance laws and that contract award will not violate any campaign finance provisions. Each affidavit is effective for one year from the date of execution, and the Commonwealth reserves the right to request updated affidavits at any time. Contractors must ensure all information is properly signed, dated, and notarized before submission.
The document includes three separate affidavit options that contractors may be required to complete depending on their status. The first is the standard campaign finance compliance affidavit required of all bidders and offerors. The second is the resident bidder status affidavit, which requires attestation that the entity is authorized to transact business in Kentucky and has maintained Kentucky corporate income taxes, unemployment insurance fund payments, and workers' compensation policies for one year prior to the bid advertisement date. The third is the qualified bidder status affidavit for nonprofit organizations furthering the purposes of Kentucky law or qualified nonprofit agencies for individuals with severe disabilities, which must employ individuals with severe disabilities for at least seventy-five percent of direct labor man hours. The bidding agency reserves the right to request supporting documentation for any claimed status and may disqualify bidders or terminate contracts for failure to provide such documentation upon request.
