About this file
This is an affidavit attachment document for the Commonwealth of Kentucky relating to bidder, offeror, and contractor requirements for state and local contract opportunities, specifically applicable to the Bus Ticketing Services procurement. The document establishes mandatory affidavit submissions required under Kentucky Revised Statutes (KRS) 45A.110 and 45A.115, with affidavits remaining effective for one year from the date of execution. All authorized representatives of contracting parties must complete notarized attestations and submit them to the Commonwealth to be eligible for contract award or renewal. The document does not specify response dates, due dates, site visits, bidder meetings, award dates, or contract terms for the Bus Ticketing Services opportunity; rather, it functions as a compliance framework requiring submission of completed affidavits prior to contract execution.
The affidavit requirements address three primary areas: campaign finance law compliance, resident bidder status, and qualified bidder status. Bidders must attest under penalty of perjury that they have not knowingly violated Kentucky campaign finance laws and that contract award will not violate such laws. Those claiming resident bidder status must demonstrate authorization to transact business in Kentucky and provide documentation of one-year prior compliance with Kentucky corporate income tax filing, unemployment insurance contributions, and active workers' compensation policies. Alternatively, bidders may claim qualified bidder status as nonprofit corporations furthering Kentucky statutory purposes or as qualified nonprofit agencies for individuals with severe disabilities, with the latter requiring employment of individuals with severe disabilities for at least seventy-five percent of direct labor man-hours. The Commonwealth reserves the right to request supporting documentation for any claimed status and may disqualify bidders or terminate contracts for failure to provide such documentation upon request.
