About this file
The document is a set of three Required Affidavits for Bidders, Offerors, and Contractors from the Commonwealth of Kentucky, dated September 16, 2022. These affidavits are legal forms designed to be completed by entities seeking to bid on or renew contracts with the state, with each form addressing different compliance requirements. The first affidavit focuses on campaign finance law compliance, requiring bidders to swear they have not knowingly violated Kentucky's campaign finance laws. The affidavit is valid for one year from the date of execution and can be requested to be updated by the Commonwealth at any time.
The second and third affidavits address specific bidder status qualifications. The Resident Bidder Status affidavit requires companies to prove they are authorized to do business in Kentucky, have filed Kentucky corporate income taxes, made payments to the state's unemployment insurance fund, and maintained a workers' compensation policy for at least one year prior to bidding. The Qualified Bidder Status affidavit is specifically for nonprofit organizations, with particular provisions for agencies serving individuals with severe disabilities, including requirements about employment percentages and organizational registration. Both additional affidavits include provisions allowing the bidding agency to request supporting documentation, with potential disqualification for failure to provide such documentation.
