B.3 Statement of Objectives

Healthcare Services Gaps in Care

1. Introduction.

The Department of Veterans Affairs (VA) through the Northern Arizona VA Health Care System (NAVAHCS) has identified challenges that exist to include inadequate access to care and gaps in care coordination for healthcare services. Inadequate access to care includes categories such as shortage of medical providers/high turnover due to location and housing challenges, lack of consistency and trust in the healthcare delivery system, large number of social & health disparities, medically underserved population, negative impacts of the digital divide, and lack of reliable transportation. The gaps in health services include categories such as primary care, mental health, women’s health, large variety of specialty care services (OBGYN, OT, PT, Audiology, Endocrinology, etc.), and ancillary services to include radiology, health education, home health, wound care, and laboratory services. Robust and forward-facing clinical providers serving in these remote settings is the best way to meet these unique needs, along with proper care coordination strategies.

The overall goal is to fill the healthcare services gaps in care. The Contractor shall provide all labor and supervision required to meet the requirements of this contract. Initially, in phase one, provide Adult, Family and Mental Health Certified Nurse Practitioners (NPs) and Mental Health specialist (Psychologist, Clinically Licensed Social Workers or Psychiatrists) to provide a range of healthcare services as outlined in the objectives section. In phase two of the task session other specialist providers will be required to such as Audiologist, Ophthalmologists, Physical Therapists, Occupational Therapists, and OB/GYN LIP (Women’s or Midwife certified APRN or MD). These services have been divided into a “phased approach” that will start out with basic care needs identified by NAVAHCS and expand into robust wellness care coordination to include Women’s Health, Physical Medicine and Rehab, sight, and hearing.

These services need to be provided via the VA clinic-in-a-clinic concept that NAVAHCS has with IHS/ tribal (638) as well as using Mobile Medical Clinics. Initial clinic locations shall include the Chinle and Kayenta clinics.
