1. BACKGROUND:

1.1. Main Telecom line: Connected by a combination of 30 plus year old copper phone line running to the Nez Perce NHS Visitor Center. This line consist of old T1 (1.5 megahertz system) which runs the computer and telecom infrastructure. Buildings affected by the old line are the Visitor Center, Green House (Resource Offices) Office, Trailer, and Maintenance Building.

1.2. Current infrastructure includes copper cable line running from the park entrance to the Telco building system. Current copper wire is 30 plus years old and is at the end of its life cycle with continued splices in wire and failing copper connections that the Telco company has stated they will not fix or upgrade.

1.3. The current telephone system is the National Parks CISCO IP Phone system that is using the plain old copper T1 (1.5 megahertz system).

2. OBJECTIVES:

This contract is to replace the old copper telecom T1 line with updated 36 Fiber Optic Cable. The end product will provide better telecommunication, intrusion and fire alarm communication, and computer systems for all the buildings located at Nez Perce National Historic Site.

3. PLACE OF PERFORMANCE:

4. PERIOD OF PERFORMANCE:

All tasks and deliverables for this contract must be completed within 60 days after Notice to proceed. Days listed herein refer to calendar days.

5. SCOPE:

The project consists of running Fiber Optic Cable from the existing installed fiber line across the highway (U.S. 95) from the visitor center that will terminate into the visitor center at Nez Perce NHS. The new 36 Fiber Optic Cable will be run in UD (1x2-1.25) conduit.

6. TASKS:

This section includes all labor, materials, tools, and other equipment, services and supervision required to complete the fiber optic cable installation work as indicated. The contractors work sequence shall include, but is not limited to:

• Laying the Fiber Optic Cable from the south side of US Highway 95 300 yards underneath the highway, park road, and parking area and connecting it to the mechanical room which contains a patch panel for the fiber cable.

• Responsible compliance and right of way access outside the park boundaries will be done before work will be started.

• In the portion where there is dirt the conduit will be plowed into place. The plow tractor will be fitted with rubber tracks in order to minimize surface disturbance. Conduit will be buried 30” to 36” deep and excess dirt will be worked back down into the trench.

• All pits will be 4 feet wide, 40 inches deep and 20 feet long, at maximum. The material will be placed to one side of the pit and then used to fill the pit once the installation is complete.

• When the cable is being installed it is ‘blown’ into the conduit and it is uncertain how far it may travel through the conduit before a pit will have to be opened up to send it further down the conduit. Contractor will co-ordinate with the COR should additional pits be required.

• During all phases of construction involving roadways, contractor will be required to ensure the public safety by implementing and utilizing proper traffic controls.

7. EXECUTION:

CONTRACTOR TASKS

A. Prior to beginning work, coordinate work schedule with the Contracting Officers

Representative (COR) and no physical work shall commence at the site until a notice to proceed is issued by the Contracting Officer.

B. Locate, identify and prevent damage to known utilities. If damage occurs, contractor shall repair utility at no additional expense to the government. Contractor is to be liable for damage to any other infrastructure within the work area while work is being performed on the project.

8. ACCIDENT PREVENTION:

i. Accident Prevention Plan (APP): Prior to beginning work contractor shall submit for approval a site-specific APP. The Contracting Officer will review the proposed program for compliance with OSHA and project requirements. If the program requires any revisions or corrections, the Contractor shall resubmit the program within 10 days.

The Contracting Officer will not issue the Notice to Proceed until the APP is approved.

The plan shall include:

1. Name of responsible supervisor to carry out the program.

2. Weekly and monthly safety meetings.

3. First aid procedures.

4. Description of each phase of the work, the hazards associated with each major phase, and the methods proposed to ensure property protection and safety of the public, National Park Service personnel, and Contractor's employees.

5. Training, both initial and continuing.

6. Planning for possible emergency situations, such as earthquakes, floods, windstorms, fires, cave-ins, slides, explosions, and power outages. Such planning shall take into consideration the nature of construction, site conditions, and degree of exposure of persons and property.

7. Waste control and potential hazardous materials precautions.

8. Personal protective equipment.

9. Hauling routes – coordinate with Contracting Officer.

ii. Ensure first aid is available and training adequate for the number of employees and the type of construction at the site.

iii. Report all accidents to the COR immediately. A reportable accident is defined as death, occupational disease, traumatic injury to an employee or the public, property damage in excess of $100, and fires. Form DI-134 must be completed and submitted to the COR within three days of an accident. Forms are available from the COR.

iv. Provide personal protective equipment that meets the requirements of OSHA, NIOSH and MSHA, Material Safety Data information where applicable, as well as ANSI.

v. Inspect personal protective equipment daily and maintain in a serviceable condition.

Clean, sanitize, and repair, as appropriate, personal items before issuing them to another individual. Inspect and maintain other protective equipment and devices before use and on a periodic basis to ensure safe operation.

vi. Provide all necessary traffic barriers, traffic control signs, markers, and flaggers when required during operations. Traffic control for occupied work areas shall be in accordance with National Standards contained in the Manual of Uniform Traffic Control Devices (MUTCD).

vii. Post specific reporting instructions for contacting Emergency Response Personnel in conspicuous locations at the work site.

viii. Identify materials considered to be “Hazardous Materials” by industry standards and provide protection and security for identified materials throughout contract period.

Hazardous materials spills shall be reported immediately and clean up shall be the sole responsibility of contractor.

ix. As a minimum, conduct weekly 15 minute "toolbox" safety meetings. These meetings shall be conducted by a foreman and attended by all personnel at the worksite. Conduct monthly safety meetings for all levels of supervision. Notify the COR so that they may attend. These meetings shall be used to review the effectiveness of the Contractor effort, to resolve current health and safety problems, to provide a forum for planning safe work activities, and for updating the accident prevention program. The COR will enter the results of the meetings into their daily log.

x. The Contractor shall assume full responsibility and liability for compliance with all applicable regulations pertaining to the health and safety of personnel and environmental management during the execution of work and shall hold the Government harmless for any action on his/her part or that of his/her employees or subcontractors, which results in illness, injury or death.

9. QUALITY ASSURANCE REQUIREMENT:

The government will periodically evaluate the contractor’s performance in accordance with the statement of work.

While performing work, the contractor shall comply with the latest applicable versions of the following:

• National Environmental Balancing Bureau (NEBB)

• Associated Air Balance Council (AABC)

• NFPA 101 (National Fire Protection Association's Life Safety Code)

• Local codes only if required by utility companies for service connections.

• National Electrical Code (NEC)

• Occupational Safety and Health Administration (OSHA) requirements

Additional Requirements:

• All affected areas must be restored to their original condition from before the work began. Any damages will be fixed and repaired at no cost to the government.

• Any unused conduit, or other misc. equipment must be removed from the worksite by the contractor and at the contractor’s expense.

10. SUBMITTALS:

The following items shall be submitted a minimum of one week prior to the Preconstruction Conference. Contracting Officer will notify Contractor of tentative date for the Pre-Construction Conference.

A. Letter designating Project Superintendent.

B. Accident Prevention Plan.

C. A list of Subcontractors for this project.

D. Satisfactory evidence of liability insurance coverage and workman’s compensation for the Contactor and all subcontractors.

E. Materials Submittals for all equipment to be installed.

11. CONTRACT CLOSEOUT PROCEDURES:

• Prior to requesting a final inspection, contractor will provide all information and diagrams of where the 36 Fiber Optic Cable is placed and terminated between the Highway 95 access point and the Visitor Center.

FINAL INSPECTION SUBSTANTIAL COMPLETION AND ACCEPTANCE

PROCEDURES

A. Request a final inspection in writing when a project or designated portion of a project is substantially complete. The Contracting Officer will proceed with the inspection within 10 days of receipt of the written request or will advise the Contractor of items that prevent the project from being substantially complete.

B. If the work is determined to be substantially complete, following the final inspection.

Contracting Officer will prepare a Punch List and issue a Letter of Substantial Completion.

C. If the work is not determined to be substantially complete following the final inspection, Contracting Officer will notify Contractor in writing. Contractor shall request a new final inspection after completing the work. Re-inspection costs may be charged against the Contractor in accordance with the Inspection of Construction contract clause.

D. Contractor shall complete the Punch List within 15 calendar days, documented weather permitting prior to requesting final inspection.

E. If Contractor completes all items of work on the Punch List and all contractually required items, Contracting Officer will issue Letter of final acceptance of work.

F. If the Contractor fails to complete the work within the time frame, the Contracting Officer may correct the work with an appropriate reduction in contract price or charge for re-inspection costs in accordance with the Inspection of Construction contract clause.
