About this file

This is a Proposer Certification Form for Request for Proposals No. 10.7.26.1 issued by David Douglas School District in Oregon for playground design and installation services. The District seeks proposals for playground design and installation work at existing playgrounds and other projects on an as-needed basis. The District reserves the right to award contracts by project and may make awards to one or more qualified firms. Proposal pricing must be honored for 90 days from the date of signature. The form requires proposers to acknowledge receipt of any addenda issued during the RFP process and to indicate their resident bidder status as defined under Oregon Revised Statutes 279A.120, which requires payment of unemployment or income taxes in Oregon during the 12 calendar months preceding bid submission, maintenance of a business address in the state, and explicit statement of resident bidder status in the proposal.

The certification form requires proposers to verify compliance with numerous statutory and regulatory requirements, including Oregon tax law (ORS 305.385), non-discrimination provisions (ORS Chapter 659 and 42 USC 2000e), and debarment and suspension certifications covering a five-year lookback period. Performance bonds and payment bonds equal to 100 percent of the contract amount are required, with submission due within 10 days of receipt of Notice of Intent to Award, along with certificates of insurance as specified in the proposal documents; however, performance bond requirements may be project-specific. Proposers must certify the absence of conflicts of interest with the District, confirm they are independent contractors as defined under ORS 670.600, and disclose any current certification from the Oregon Office of Minority, Women & Emerging Small Business (OMWESB). All proposal costs are the sole responsibility of the proposer, and no reimbursement will be provided by the District.
