About this file

This is a Past Performance Questionnaire template designed for customers to evaluate Architect-Engineer (A-E) firm performance on completed projects. Firms responding to the SF-330 solicitation for Project 637-25-152 (Correct Mechanical & Plumbing Deficiencies at Charles George VA Medical Center) must obtain completed questionnaires from at least three reference projects identified in their SF-330 Section F. Customers must submit completed questionnaires directly to Government Contract Specialist Chanel Johnson (chanel.johnson@va.gov) and Contracting Officer Keith Hunter (keith.hunter@va.gov) by 8:00 AM EST on February 10, 2026, not to the A-E firm itself.

The questionnaire evaluates A-E performance across 21 rated factors using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or N/A). Rating categories cover design services including personnel competence, responsiveness, consultant coordination, cost estimation, and schedule compliance; follow-on construction support services such as submittal review and design issue resolution; and where applicable, LEED documentation and BIM coordination. Additional evaluation areas address overall cost control, management performance, and technical quality, with a yes/no question regarding future engagement. Evaluators must provide narrative explanations for any Marginal or Unsatisfactory ratings and report the number of design errors and omissions and associated cost increases. The questionnaire requires evaluator information including company name, title, telephone, and email address.
