About this file
This is a Statement of Work (SOW) for occupational medical examination and fitness determination services for the Department of Transportation's Protective Service Division (PSD), Occupational Series 1801. The U.S. DOT, Office of Intelligence, Security, and Emergency Response (S-60), seeks a qualified contractor to provide nationwide medical examination services for PSD applicants and employees to ensure they meet Federal 1811 Law Enforcement Medical Standards and remain medically fit to perform safety-sensitive duties, including carrying firearms, operating government vehicles, and executing protective operations. The contractor must deliver pre-employment exams, periodic employee exams, return-to-duty reviews, fitness-for-duty evaluations, and medical clearance determinations using standardized categories (Cleared, Unable to Clear, Not Cleared) within five to seven business days of receiving complete records. Medical examinations shall include comprehensive components such as medical history review, physical examination, vital signs, vision and hearing testing, laboratory work, EKG, spirometry, urinalysis, and chest imaging as medically necessary.
The contract encompasses a 12-month base period with four optional 12-month renewal periods for a total of five years. The contractor must maintain nationwide medical exam locations accessible to employees and applicants, employ qualified licensed medical professionals including a Medical Review Officer or equivalent physician, designate a program manager as primary point of contact, and implement secure systems for medical records management and status tracking. Estimated workload includes approximately 15 initial pre-employment exams during a staffing surge phase followed by approximately 10 periodic annual exams upon reaching full staffing of 30 positions, with additional volume for return-to-duty and fitness-for-duty evaluations based on turnover. Deliverables include written medical determinations, bi-weekly or monthly status reports, secure online status tracking, and monthly itemized invoices. The Government retains final authority over all medical waivers and complex "Unable to Clear" determinations, with a designated Contracting Officer's Representative providing oversight.
