About this file
This is an Attachment A form for RFP 2026-RFP-070-PR issued by a Wisconsin county for the acceptance of hazardous waste for disposal. The document serves as a Disposal Methods and Facilities Declaration that bidders must complete as part of their proposal submission. Bidders are required to specify disposal facilities for each of 32 waste categories, including oil-based paints, halogenated solvents, flammable liquids, acids, alkalines, toxic liquids and solids, oxidizers, mercury and mercury-containing materials, PCB-contaminated items, compressed gas cylinders, antifreeze, asbestos, vaping devices, and used oil filters. For each waste category, bidders must identify both a primary disposal facility and a secondary facility. All specified facilities must be selected from a pre-approved facilities list referenced in Section 4.9 of the RFP.
Bidders are prohibited from making any changes to the recycling or disposal methods and facilities specified in their declaration without prior written approval from the Hazardous Waste Coordinator. The use of any disposal facility is contingent upon prior approval by the Hazardous Waste Coordinator. This document represents a critical component of the proposal evaluation process, as it demonstrates the bidder's capability and capacity to manage the full range of hazardous waste streams the county generates.
