About this file
This document is an Addendum to FAR 52.212-1 providing instructions to offerors for submitting quotes on commercial products and commercial services. Quotes must be submitted electronically by a specified deadline, with late submissions ineligible for consideration. The quote submission must include three volumes: (I) Technical/Product Compliance, limited to 2 pages excluding product literature, requiring manufacturer name, product name, part/model number, product literature/cut sheets, confirmation of compliance with Attachment 1, and OEM authorization letter if applicable; (II) Price with no page limit, requiring a completed CLIN pricing schedule with firm-fixed prices for all items including shipping, handling, or installation costs, and if the price is lower than market price, a letter explaining the reason for the lowest price; and (III) Representations with no page limit, requiring active SAM registration and completed FAR 52.212-3 representations if not current in SAM.
The document emphasizes that quoters must provide sufficient product information for government verification of technical acceptability and cannot simply restate government requirements or state "will comply." Only compliant quotes that meet all terms, conditions, and specifications in the RFQ will be evaluated for award. Quoters must be original equipment manufacturers, authorized dealers, authorized distributors, authorized resellers, or authorized service providers for proposed medical supplies, medical equipment, and/or maintenance services contracts, verified by OEM authorization documentation. All software licensing, warranty, and service must comply with OEM terms and conditions.

Quoters are cautioned that simply restating the Government requirement or stating “will comply” may be insufficient. The quote must provide enough product information for the Government to verify technical acceptability. A compliant quote is one that meets all of the terms, conditions, and specifications in the RFQ. The Quoter must provide all documents listed and comply with all RFQ instructions in order for their quote to be considered “compliant” with this RFQ. Only compliant quotes will be evaluated for award.
Notes: Quoter shall be an original equipment manufacturer (OEM), authorized dealer, authorized distributor, authorized reseller, or authorized service provider for the proposed medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment (i.e., replacement parts), verified by an authorization letter or other document(s) from the OEM, such that the OEM’s warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions
