About this file

This document is Appendix C of the USDA Forest Service TES Plant Survey Field Guide, containing a standardized field form template for conducting Threatened, Endangered, and Sensitive (TES) plant surveys on federal forest lands.

The form captures comprehensive survey documentation across four main sections. The General Information section requires surveyors to record a survey ID, name, status, source of work, survey type, focus area, estimated acreage, elevation data (minimum, maximum, average), and ecological parameters used to direct the survey. Survey visit documentation requires recording visit dates in MM/DD/YYYY format and the names of examining personnel for each field visit. The Target Species section mandates listing all targeted plant species using NRCS PLANTS codes and scientific names, with required fields indicating whether suitable habitat was found and whether the plant was actually located, along with corresponding FS Site IDs if Element Occurrence (EO) forms were completed. The Species List of Surveyed Area is optional and documents other species encountered during the survey, including their NRCS code, scientific name, life form, habitat type, percent cover or cover class, and notation of non-native species. The Optional Location Information section provides fields for recording geographic context (state, county, region, forest, district), USGS and Forest Service quad numbers, legal descriptions using the public land survey system, GPS coordinates with datum specifications, GPS unit manufacturer and model details, directions to the survey area, and a sketch box for visual representation of the survey boundaries.
