About this file
This is a Past Performance Questionnaire template document designed for contractors bidding on construction services at Lake Baldwin Site for the Department of Veterans Affairs Veterans Health Administration, Veterans Integrated Service Network 8.
The questionnaire serves as an evaluation tool for assessing offerors' qualifications and historical performance on similar projects. As an appendix to the solicitation 36C24826Q0585 for addressing site deficiencies at Building 3132, this document functions as a standard template rather than containing specific substantive requirements. Contractors responding to this solicitation with a contract value between $1,000,000 and $5,000,000 are required to complete the past performance questionnaire as part of their proposal submission. The questionnaire will be used to evaluate contractor experience, capability, and track record in completing comparable construction projects. Offerors must be registered in SAM.GOV, certified as Service-Disabled Veteran-Owned Small Business (SDVOSB), and verified in the SBA's VetCert program database. Quote submissions are due by the deadline specified on the SF1442 form, with all questions regarding the solicitation due by Monday, May 18, 2026, at 11:00 AM EST, and a mandatory site visit scheduled for Wednesday, May 13, 2026, at 10:00 AM EST.
