About this file

This is a Past Performance Questionnaire template for the Department of Veterans Affairs' competitive enterprise-wide strategic contract for in-home hospital beds (Solicitation #36C10G25R0011). The questionnaire must be completed by references and submitted to specified VA personnel via email.

The questionnaire evaluates contractors on a 1-5 scale across multiple performance areas including overall quality/satisfaction, delivery performance, quality of service, problem resolution, and quality of contractor personnel. It also includes questions about any cure notices, terminations for default, option exercises, contract values, and performance periods. The form requires details about adverse performance issues and corrective actions, and asks whether the reference would award future contracts to the contractor. References must indicate if past performance information is available in searchable databases and can attach additional performance reports or materials to support their evaluation.
