About this file
This is a routine inspection report for the Williams River Bridge (Structure Number 092104000001010), a concrete slab bridge carrying Forest Road 135 over the Williams River in West Virginia, inspected on July 29, 2025, by the U.S. Forest Service.
The bridge, built in 1950 and located near Tea Creek Campground at mile point 0.05, is rated in Fair condition (rating F) with a lowest component condition rating of 5. The 111-foot structure consists of five concrete slab spans with a 13.5-foot curb-to-curb width, carries an estimated 25 annual average daily traffic, and is posted for load (30.0-39.9 percent below HL-93 design capacity). Key deficiencies identified include widespread CS2 moderate cracking throughout all slabs, minor delamination and spalling on the deck, approximately 4 feet of scaling on abutments and piers, and a large spalled area with exposed rebar on the upstream fascia beam. The substructure rated Satisfactory (6) with no undermining detected. Critical vulnerabilities include a very high overtopping likelihood (once every two years or more frequently), scour criticality with temporary countermeasures in place, and poor approach roadway alignment. Load ratings indicate an operating rating of 0.77 and inventory rating of 0.59 under LRFR methodology. The inspection team noted the bridge is scheduled for replacement in the 2026-2027 construction season with a new, higher structure constructed upstream, followed by full demolition of the existing bridge. A safety advisory warns that large wolf spiders inhabit all pier and abutment caps.
