About this file
This is a Professional Questionnaire for the Michigan Department of Environment, Great Lakes, and Energy (EGLE) Remediation and Redevelopment Division for Operation and Maintenance Services at the Former Ott/Story/Cordova Superfund Site in Muskegon, Michigan. The questionnaire requests comprehensive information from prospective contractors regarding their qualifications to manage a groundwater extraction and water treatment system at the contaminated site located at 453 Agard Road, Muskegon, MI 49445. Respondents must demonstrate at least three years of prior experience with Federal Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) sites and Michigan NREPA Part 201 contaminated sites, with emphasis on technical support and operation and maintenance services. The questionnaire requires detailed descriptions of experience with water remediation systems, groundwater extraction systems, analytical laboratory operations, monitor well sampling, and compliance with NPDES monitoring requirements.
The questionnaire mandates that responding firms identify three key personnel: an Operations Manager who must hold a current Class A wastewater treatment operator license with specific industrial certifications and maintain full-time on-site presence with one-hour emergency response capability; a Laboratory Chemist with a bachelor's degree in chemistry and minimum five years of laboratory management experience; and an Engineer/Project Manager with a college degree in engineering and minimum ten years of project management experience. Firms must provide organizational charts, resumes for key personnel, evidence of HAZWOPER training completion for all team members, regulatory knowledge of applicable Michigan and Federal environmental statutes, and information regarding any sub-consultants or subcontractors with letters of intent. The document also requests identification of special qualifications, innovations, awards, or recognitions that would demonstrate the firm's capability to perform the ongoing operation and maintenance services for this Superfund remediation site.
