About this file
This document is a Life Safety Risk Assessment Tool and related assessment templates used to evaluate and manage life safety code deficiencies in buildings during construction or maintenance activities.
The Initial Life Safety Risk Assessment Tool (Attachment 3) provides a framework for identifying and scoring building deficiencies based on two primary factors: severity of occurrence and impact of the deficiency. Severity is categorized into four levels, from Category 1 (deficiency likely to cause major injury or death) to Category 4 (minimal safety impact). Impact is assessed across four scopes: Facility Wide, Multiple Units/Floors, Local/Single Unit, or Short Duration. By cross-referencing these two factors in a matrix, assessors determine a Risk Tolerance Score of HIGH (requires specific remediation actions), MEDIUM (requires remedial action), LOW (no ILSM required), or No ILSM Required. If a HIGH or MEDIUM score is assigned, a Full Interim Life Safety Measures Assessment (Attachment A.1) must be completed.
Attachment A.1 identifies specific life safety code deficiencies that may trigger the need for Interim Life Safety Measures (ILSM), including issues such as lacking code-complying smoke barriers, improper fire exit stair discharge, excessive travel distances to exits, lacking two remote exits, nonconforming building construction, improperly protected vertical openings, large fire barrier penetrations, unprotected hazardous areas, non-functioning fire/smoke dampers, blocked exits, rerouted emergency room traffic, occupied floor renovations, and disconnected alarm devices. Attachment A.2 provides a decision matrix mapping specific deficiencies to appropriate ILSM interventions, which include posting alternative exit signage, daily egress inspections, temporary fire alarm systems, additional firefighting equipment, temporary construction partitions, hazard surveillance, combustible load control, firefighting training, and quarterly fire drills. Attachment A.3 finalizes the implementation plan by identifying which ILSM measures will be executed and designating responsible parties, requiring sign-off from the Contracting Officer's Representative, Engineering Service Chief, and Construction Safety Committee Chair.
