J-5 Past Performance Questionnaire Cover Letter

The Washington Headquarters Services Acquisition Directorate is requesting proposals for a solicitation in support of the Office of the Under Secretary of Defense Acquisition and Sustainment (OUSD (A&S)) Office of the Deputy Assistant Secretary of Defense for Logistics located at the Pentagon in Washington, DC. The solicitation is for the acquisition of Non-Tactical Vehicle Fleet Management and Logistics support services. We require all offerors to provide past performance information.

Moreover, we request your assistance by completing and forwarding the attached past performance questionnaire to the Washington Headquarters Services Acquisition Directorate via email to benjamin.j.juricic.ctr@mail.mil and cc the Contracting Officer at patricia.a.watson30.civ@mail.mil.

To submit via email, subject line should read: Past Performance Questionnaire, Solicitation HQ0034-20-R-0206

2. In order to ensure the questionnaire is received at our office in time, please forward this questionnaire to arrive no later than 25 June 2020.
