About this file
This is a Standard Operating Procedure (SOP) document from the VA Healthcare System's Radiology Service in Orlando, FL dated March 2021, which establishes protocols for handling critical tests and critical results in radiology. The document defines critical tests as time-sensitive imaging examinations requiring priority review, and critical results as findings that could result in mortality or significant morbidity if urgent follow-up is not taken.
The SOP details 11 specific critical results (including tension pneumothorax, ruptured aortic aneurysm, acute intracerebral hemorrhage, etc.) and 2 critical tests (intra-operative radiographs for retained foreign bodies and acute stroke protocol CT). For critical results, radiologists must report findings in the radiology report, set alert code 1002, and contact the ordering practitioner within 60 minutes by phone or in person. For critical tests, particularly STAT intra-operative radiographs, findings must be reported to the surgical team within 30 minutes. The document includes specific documentation requirements, including recording who was informed, when, and by what method, with a standardized format for documentation. This SOP appears to be attached to a solicitation for teleradiology services at the Orlando VA Healthcare System.
