About this file

This is an attachment containing appropriation certification requirements and compliance certifications for a federal grant application (W9127N-26-2-R001).

The document outlines two primary certification requirements that applicants must acknowledge by checking "I Agree" on either the SF 424 Block 21 or SF 424 (R&R) Block 17. First, applicants must certify that all statements in the application are true, complete, and accurate to the best of their knowledge, and that they understand false or fraudulent statements may result in criminal, civil, or administrative penalties under U.S. Code Title 218, Section 1001. Additionally, applicants must certify that their organization does not require employees, contractors, or subrecipients to sign confidentiality agreements that would restrict lawful reporting of fraud, waste, or abuse to federal investigative or law enforcement representatives. Second, the document includes a Certification Regarding Environmental Tobacco Smoke under Public Law 103227 (Pro Children Act of 1994), which requires applicants to comply with smoking prohibitions in federally-funded indoor facilities that provide health, day care, education, or library services to children under age 18. Non-compliance may result in civil monetary penalties up to $1,000 per day and/or administrative compliance orders. Applicants must also require all subgrantees to include and certify compliance with these same provisions. The certification requires signature by a duly authorized representative with printed name, title, and date.
