About this file
Summary
This is a certification template (Attachment 5) from federal grant documentation (W9127N-26-2-R001) that outlines appropriation certification requirements applicants must acknowledge when submitting applications. By checking "I Agree" on the SF 424 or SF 424 (R&R) form, applicants certify that all statements in the application are true, complete, and accurate to the best of their knowledge, and agree to comply with resulting award terms. Applicants also acknowledge awareness that false, fictitious, or fraudulent statements may result in criminal, civil, or administrative penalties under U.S. Code Title 218, Section 1001. Additionally, applicants must certify that their organization does not require employees, contractors, or subrecipients to sign confidentiality agreements that would restrict them from lawfully reporting fraud, waste, or abuse to designated federal investigative or law enforcement representatives.
The template includes a specific certification regarding compliance with Public Law 103-227 (the Pro Children Act of 1994), which prohibits smoking in federally funded indoor facilities used routinely for health, day care, education, or library services to children under 18. Non-compliance can result in civil monetary penalties up to $1,000 per day and/or administrative compliance orders. Applicants must agree to include this certification language in any subawards containing children's services provisions and require subgrantees to certify accordingly. The form requires signature, date, applicant name, and the printed name and title of the authorized representative executing the certification.
