ADDENDUM TO FAR 52.212-1 -- INSTRUCTION TO OFFERORS, COMMERCIAL ITEMS

Tailoring of 52.212-1: FAR 52.212-1(c), Period of acceptance of offers, is amended to read 60 days versus 30 days.

The offeror shall make a clear statement in Volume 1 that the offer is valid for 60 days after the solicitation closing date.

Notice to Offerors: Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.

1. Instructions to Offerors-Commercial Items.

a. General Instructions.

(1) To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of two (2) separate proposal volumes; Part I – Price, Part II – Past Performance.

(2) ONLY THE MINIMUM AMOUNT OF INFORMATION REQUIRED TO PROVIDE FOR PROPER

EVALUATION IS REQUESTED. EFFORTS SHOULD BE MADE TO KEEP OFFERS AS CONCISE AS

POSSIBLE WITH THE EMPHASIS BEING ON PROVIDING INFORMATION ESSENTIAL FOR PROPER

EVALUATION. The proposal shall be clear, concise, and include sufficient detail to provide for adequate evaluation and for substantiation of the validity of stated claims. Submitted documents must contain adequate information to enable the Contracting Officer to fully ascertain offerors capability to perform contract requirements, must meet the requirements of the RFP, and must address the evaluation factors for award.

(3) Offerors shall acknowledge awareness of all amendments to the solicitation by signing and dating Page 1 of each Standard Form 30, Amendment of Solicitation/Modification of Contract, and including it in proposal Volume I, Price.

(4) The Contracting Officer has determined there is a high probability of adequate price competition for this acquisition so offerors are cautioned to submit their best pricing up-front.

b. Pre-Proposal Site Visit. There will NOT be a pre-proposal site visit for this requirement. Attachments 1, Custodial Performance Work Statement, Attachment 2, Building Floor Plans, Attachment 3, Fixture Count by Facility can be used in developing your proposal.

2. Proposal Preparation Instructions.

a. To assure timely and equitable evaluation of offers, offerors must follow the instructions contained herein.

Offerors are required to meet all solicitation requirements, including terms and conditions, and representations and certifications, in addition to those identified as evaluation factors. Failure to meet a requirement may result in an offer being deemed non-compliant and ineligible for award. By submission of its proposal the offeror agrees to hold the prices firm for 60 calendar days from the solicitation closing date.

b. Proposal Preparation Instructions. Failure to furnish a complete proposal may result in a determination by the government that the proposal is non-compliant. Electronic Copies: Proposals shall be submitted in a digital format in one original copy to arrive by the date and time specified in the solicitation. Only electronic/digital copies of proposals will be accepted. Faxed, hand carried, or mailed proposals will NOT be accepted. Send electronic copies of each volume via email to TSgt Jerime Gorse at jerime.gorse@us.af.mil AND to MSgt Michael McDaniel at michael.mcdaniel.7@us.af.mil by the date and time specified in Block 8 of the SF1449.

1. General Format and Content. The offeror shall submit the following volumes of material. A complete proposal consists of one volume:

Volume I – Price

Specific guidance regarding format and content is discussed in the following paragraphs. The Government will only read/evaluate the maximum number of pages allowed. All pages provided by the offeror in excess of the stated limitations will be returned to the offeror unread. The proposal content shall be formatted as follows:

1) Text: Single-spaced

2) Font Size: Microsoft Office Word, Times New Roman, 12 font

3) Margins: One Inch (1”) left, right, top and bottom

4) Paper Size: 8½” x 11”

5) Page Numbering: Sequential (i.e., 1 through 100)

Note: The font size requirement does not apply to charts, however the font size must be reasonable and easily readable. Also, page numbering must be sequential throughout the entire document. At the beginning of a new chapter or section, do not restart the page numbering.

Legible tables, charts, graphs, and figures shall be used wherever practical to depict organizations, implementation schedules, plans, etc. These displays shall be uncomplicated, legible, and shall not exceed 11 x 17 inches in size. 11 x 17 pages may only be used for large tables, charts, graphs, diagrams, and schematic – NOT for pages of text.

Cover Sheets: Each volume shall include a cover sheet with the following information:

1) Volume number

2) Solicitation number

3) Name, address, email, and telephone number of the offeror DUNS and CAGE number

4) A statement of acknowledgement shall be included for each amendment, if applicable.

5) Volume I cover sheet shall include a statement that the offer and prices will remain in effect for 60 days after the solicitation closing date.

Elaborate formats or color presentations are not desired or required.

2. Instructions for Volume I – PRICE: SF 1449, Bid Schedule, Representations and Certifications, and any parts of the request for proposal with fill-in blanks.

a. Standard Form (SF) 1449: Complete blocks 12, 17a, and 30a, b, and c of the SF1449. An official having the authority to legally bind the company contractually must sign and date the SF1449. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation, with attachments. Failure to acknowledge any amendments to the solicitation can result in an offeror being deemed non-compliant.

b. Bid Schedule: Insert proposed unit and extended prices in the Bid Schedule (Attachment 5) for each Contract Line Item Number (CLIN) for the base period and all option periods. For ease of administration the unit and total price should be rounded to the next whole dollar amounts.

c. Organization Chart: Submit a one (1) page chart showing the staffing approach which must consist of at a minimum the Contract Manager/Program Manager, Contract Site Lead/Site Superintendent, and labors. Names are not required to be the chart.

d. Representation and Certifications: Complete the required Representation and Certifications or indicate if the information is contained in the System for Award Management (SAM) located at https://www.sam.gov.

3. Instructions for Past Performance:

a. Nothing is required from the offeror for Past Performance.

b. The Government will pull Contractor Performance Assessment Reporting System (CPARS) data from the last three (3) years for each offeror.

c. The Government will also pull Federal Awardee Performance and Integrity Information System (FAPIIS) data on each offeror.

ADDENDUM TO 52.212-2

Addendum to FAR 52.212-2:

1. BASIS FOR CONTRACT AWARD:

a. This is a competitive best value determination in which competing Offerors' price and past performance will be evaluated. In order to be eligible for award, offerors are required to meet all solicitation requirements, such as terms and conditions; representations and certifications; evaluation factor requirements; and be deemed responsible in IAW FAR Part 9. If an offeror fails to meet all solicitation requirements, the Government may not make an award to that offeror.

b. Proposals will be evaluated using the following two (2) factors:

Factor 1: Price Factor 2: Past Performance

c. Evaluation Process:

1) Proposals will be checked for completeness as specified in the Addendum to FAR 52.212-1, Instructions to Offerors.

2) Complete proposals will be ranked from lowest price to highest Total Evaluated Price (TEP). The TEP will be determined by adding the base period, all option periods and the FAR 52.217-8 six month extension option. Evaluation of options shall not obligate the Government to exercise such options.

3) The Past Performance Evaluation will be conducted by the Government on all offerors.

4) During the Past Performance evaluation the government will take all CPARS ratings (Overall Ratings) from the last three (3) years and average them together to come up with the Past Performance Rating. For Example: Offeror A has three (3) CPARS ratings in the last three (3) years, two (2) “Very Good’ ratings and one (1) “Satisfactory’ rating. When averaged they will receive a “Very Good’ rating for Past Performance.

5) IAW with FAR 13.106-2, Evaluation of quotations or offers, Where an evaluation is based only on price and past performance, the Contracting Officer will make an award based on whether the lowest priced offer having the highest past performance rating possible represents the best value when compared to any lower priced offer.

d. The Government intends to award a contract based on proposals received without further discussions or clarification.

2. Evaluation Factors for Award Two evaluation factors will be used to evaluate proposals: price, and past performance. Price and past performance evaluations will be performed based on the following criteria:

a. Price Price will be evaluated to ensure the Government receives a fair and reasonable price. The Government will evaluate offers for award purposes by adding the total price for all CLINs to include the base period, all option periods, and 50% of the last option period to evaluate the option to extend services per FAR 52.217-8. This will constitute the Total Evaluated Price (TEP). Evaluation of options shall not obligate the Government to exercise such options.

b. Past Performance

1. Evaluation. Past performance will be evaluated based on all CPARS ratings (Overall Ratings) from the last three

(3) years. The Government will average the ratings from the last three (3) years together to come up with the Past Performance Rating. For Example: Offeror A has three (3) CPARS ratings in the last three (3) years, two (2) “Very Good’ ratings and one (1) “Satisfactory’ rating. When averaged they would receive a “Very Good’ rating for Past Performance.

NOTE: In the case of an offeror without any past performance (CPARS ratings) or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance.

Therefore, the offeror shall be determined to have unknown (or “neutral”) past performance.
