ASBESTOS INSPECTION REPORT INFORMATION
This inspection was performed in accordance with the Utah Division of Air Quality (DAQ) requirements as found in the Utah Air Quality Rule R307-801-10. This inspection report is required to be on site during all abatement, renovation, and demolition activities. Samples referenced in this report were analyzed by polarized light microscopy (PLM) utilizing method 600R-93-116 by The Science and Engineering Laboratory (AIHA Acc.#-
101572) at Hill Air Force Base, Utah (R307-801-10.8.a-b).

Civil Engineering personnel also reviewed previous asbestos inspection reports of suspect asbestos containing materials (ACM) that could potentially be encountered in the proposed area/areas. The information gathered from all current and previous inspections is shown below by homogenous area (R307-801-9-4).
The quantities within this report are estimates and are not to be used for bidding purposes.

RACM: Regulated Asbestos-Containing Material (RACM)" means friable ACM, Category I non-friable ACM that has become friable, Category I non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading, or Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition or renovation project operations.
Locations found: Locations of building materials as described in this report indicate where they were found, but do not necessarily mean that these are the only locations where these materials may be encountered during the project.
Category II Non-Friable ACM: Any material, excluding Category I non- friable ACM, containing more than 1% asbestos as determined by using the methods specified in 40 CFR Part 763, Subpart E, Appendix E, Section 1, Polarized Light Microscopy (PLM) that, when dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure.

MATERIAL TYPE
Per Hill AFB requirements, all materials containing any detectable amount of asbestos shall be handled as Asbestos Containing Material (ACM). Table 2 shows the summary of materials containing ≤1% that were identified via "Point Counting". Per EPA/UDAQ
≤1% is not considered to be an asbestos containing material, but special handling/packaging requirements are necessary per OSHA 29CFR 1926.1101 and
29CFR 1910.1001

5-This inspection report only encompasses the areas/materials designated within the scope of work that was provided at the date of the inspection (see title page for date). This report must be modified should the scope of the project be altered in any way or additional materials not previously identified within this report are encountered. This report may not be used for any other projects within the building.
LIMITED LEAD-BASED PAINT IDENTIFICATION REPORT INFORMATION
Table 1.
2-The condition column is the condition only of the painted/coated component (See final page for all readings/samples).
3-The U.S. Department of Housing and Urban Development defines Lead-based paint as any paint, varnish, stain, or other applied coating that has 1 mg/cm² as measured by an X-ray Fluorescence (XRF) Analyzer or laboratory analysis or 0.5 percent by weight (5,000 µg/g by dry weight) by laboratory analysis, or more of lead. All other components tested were less than 0.80 mg/cm².
4-Any effort to disturb lead paint can create lead dust. Ensure that appropriate abatement, cleanup, and disposal will be accomplished and that appropriate safety measures are taken IAW 29 CFR 1926.62. If you have any questions concerning this report, contact the Asbestos/LBP Shop: Taylor Brimberry at 586-7094.

ENVIRONMENTAL RESTORATION (AFCEC/CZOM)
AF Form 813 Review Requestor: James Morris, (777-3423) 75 CEG
813 ID: 40291
Installation Restoration Program (IRP) Comments:
There is no known surface contamination beneath or adjacent to Buildings 483. However, any construction project has the potential to uncover previously undocumented contamination. If unusual soil stains or odors are encountered during any excavation that may be required for this project, or if you have questions regarding the wells, please contact the Restoration POC, Mr.
Lindsay Burt (775-3301).
Environmental Restoration funds cannot be used to address contamination discovered during a construction project (MILCON or non-MILCON) per Section 6.4 of AFI 32-7020 (7 Nov 2014). If a construction project generates actions that result in the need to address contamination, or a need to change Environmental Restoration Program timelines to address contamination, the costs of such actions are not eligible for the use of Environmental Restoration funds and shall be funded as part of the construction project. This includes the handling, mitigation, and disposal or other disposition of contamination discovered before or during the construction activity. Excavations that result in the need for soil disposal will either dispose of clean soil at the on-base Construction and Debris landfill or use as fill for another on-base project.
Contaminated soil will be disposed of at a permitted landfill.

HILL AIR FORCE BASE
Construction Project Water Quality Compliance Planning CHECKLIST hafb_construction_site_checklist_24march2021.docx PRINTED: 24 March 2021
Instructions - This checklist should be used by the design engineer to determine the necessary design considerations and the submittals for a Stormwater Pollution Prevention Plan (SWPPP) and/or Erosion Control Plan (ECP) review. Once submitted by the Hill AFB PM, it will be used by 75 CEG/CEIE to review and approve these documents. (**This checklist is not all inclusive; consult with CEIE for site-specific issues.)
General The attached form is a list of typical stormwater review criteria for all construction projects.
o The Design Engineer should check each item on the list; if an item is not applicable to this project, the item should be noted as N/A o Upon submittal for plan review, the Design Engineer should sign this form indicating the checklist and all required documents have been submitted o Hill Air Force Base (AFB) Project Manager (PM) must review documents from the Design Engineer and check what is included with the packet and sign this checklist prior to submittal to 75 CEG/CEIE o A copy of this checklist must accompany the plans with all subsequent reviews, 75 CEG/CEIE will not accept plans for review without this signed checklist.
o In providing approvals, 75 CEG/CEIE assumes that applicants have not made any errors and have complied with all applicable regulations. If after an approval, an error is discovered, or it is discovered that some aspect of approved drawings does not comply with applicable regulations, the applicant shall, at their own expense, revise the drawings and modify any infrastructure as necessary to correct the problem. Applicants and their design professionals shall remain responsible for their projects at all times.
The Hill AFB PM is responsible for compliance with all Department of Defense (DoD), Air Force Instruction (AFI), Unified Facilities Criteria (UFC), and State of Utah Division of Water Quality (DWQ) requirements. In addition, the Hill AFB PM is responsible for compliance with the DoD Implementation of Stormwater Requirements under Section 438 of the Energy Independence and Security Act (EISA) and obtaining all necessary permits and approvals.
