About this file
This is a government facility security review form (Attachment 6) used to document the results of security compliance inspections. The form provides three checkbox options for the government reviewer to indicate their findings: 1) No security concerns identified and contractor/sub-contractors meet security requirements, 2) Security concerns were identified but resolved by the contractor with no further action needed, or 3) Security concerns were identified and referred to the Contracting Officer for resolution.
The form includes space to indicate whether additional comments are attached and requires the government reviewer's name, signature, date, and telephone number. This appears to be a standard template used for documenting facility security compliance reviews related to the court reporting services contract for the U.S. Attorney's Office in the Northern District of Oklahoma.
