About this file
This is a Past Performance Summary Sheet template used to evaluate contractor performance on relevant past contracts for federal contract opportunities. The form requires offerors to submit completed sheets for contracts they consider relevant to the proposed effort, demonstrating their ability to perform the work. Offerors must limit submissions to the number requested in the solicitation and provide concise performance comments, including rationale for relevance, scope, magnitude, complexity, recency, customer satisfaction metrics, and quality of service documentation. The template requests specific contract information including program title, contractor name and CAGE code, contract number and type, competitive award status, small business set-aside designation, period of performance, initial and final contract costs with explanations for any cost variances, and whether the offeror performed as prime contractor or subcontractor with a percentage estimate of work performed.
The form also requires detailed points of contact information for three primary customer representatives: the Program/Site Manager, Contracting Officer, and Administrative Contracting Officer, including their office locations, telephone numbers, and email addresses. Offerors must clearly delineate the nature and portion of work they performed on each referenced effort and explain how that experience relates to the proposed scope of work. The template is designed to be submitted separately for each contract referenced and applies to government contracts as well as commercial contracts with equivalent points of contact.
