About this file

This document is an attachment to a Request for Proposal (RFP) for a federal contract opportunity. It is the RFP Questionnaire that offerors must respond to in order to be fully considered for the award.

The key details of the RFP Questionnaire are:

  1. Contractor's Experience: Offerors must demonstrate their recent experience (within the past 6 years) performing various common forest road construction activities, such as excavation/embankment, retaining structures, culvert installation, concrete box culverts, sub-drain systems, road aggregate, minor concrete structures, erosion/siltation protection, and simple span bridge structures. Offerors will be evaluated based on the complexity, dollar value, and contact information for the projects they have completed.
  2. Contractor's Project Management: Offerors must explain their company's capabilities to provide project management, on-site supervision, quality control, and manage multiple ongoing projects.
  3. Contractor's Past Performance History: Offerors must ensure that at least three recent (within 6 years) past performance questionnaires (Attachment 6) are completed by their references and returned with their offer, or provide current (within 6 years) Contractor Performance Assessment Reporting System (CPARS) reports for relevant contracts.

The related federal contract opportunity is for an Indefinite Delivery/Indefinite Quantity (IDIQ) contract for Western Oregon Road Construction, solicited by the Department of Agriculture Forest Service R6-Pacific Northwest Region.

REQUEST FOR PROPOSAL

QUESTIONNAIRE

OFFERORS must respond to the following to receive full consideration for award.

SEE SOLICITATION SECTION M for EVALUATION CRITERIA.

1. CONTRACTOR'S EXPERIENCE

On separate pages (ONE (1) page limit per road construction activity; subsequent pages will not be read. If photos for a particular road construction activity are being provided, one additional page, per road construction activity, is permitted), demonstrate your recent experience (within the past six years) performing each of the following common forest road construction activities by providing information regarding (a) name and description of the project performed, (b) the complexity of the project performed, (c) the dollar value of the project performed, (d) the dates of performance, (e) current contact information for the project COR, or owner, who can verify the stated experience, (f) project photos, if available. Experience will be evaluated on the basis of the jobs that the offeror has had similar in nature, scope, and complexity to the work that the contractor will have to perform under the contracts awarded based on this solicitation. The more experience items that an offeror has performed on past projects, the higher the experience rating that will be given. Contractors with demonstrated experience performing three or more of the common forest road construction activities listed will be rated higher than contractors who have performed less. If an offeror has no experience with similar work, the government will evaluate its experience as “None” for that item which would be an unfavorable assessment.
