About this file

This is a Quality Assurance Surveillance Plan (QASP) Matrix for noxious weed survey and treatment services. The document establishes performance standards and quality metrics for contractor delivery of daily treatment records, electronic data collection, and submission activities across all treatment methods.

The QASP defines three quality rating levels tied to specific performance thresholds and corresponding payment adjustments. Rating 1 (Acceptable) requires 90% or better treatment efficacy on targeted plants, 20% or less loss of non-target species, complete and accurate treatment records submitted timely, and electronic data meeting 90% accuracy or greater. Rating 2 (Deficient) applies when treatment efficacy falls between 80-90%, triggering mandatory contractor rework at the contractor's expense; if standards are met after rework, full payment is made; if not met after a second inspection, a 20% payment reduction applies. Rating 3 (Unacceptable) results in no payment when less than 80% of quality standards are met or when treatment records and electronic data are not provided. Additional penalties include 10% payment reduction for late or incomplete submission of treatment records and electronic data. Monitoring methods include weekly review of Quality Control Plan documents by the Contracting Officer's Representative (COR), periodic onsite inspections by inspectors or COR, final treatment efficacy inspections at least 14 days after completion using ocular estimates and monitoring plots as needed, and office review of all submitted documentation.
