About this file

This document is a Quality Assurance Surveillance Plan (QASP) for maintenance and support services of the Public Address Mass Notification System (PAMNS) and Code Blue Help Point systems at the VA Salt Lake City Healthcare System. The QASP establishes systematic evaluation procedures to ensure contractor compliance with the Statement of Work and maintains continuous operation and reliability of both systems. The government retains responsibility for objective, fair, and consistent performance evaluation, while the contractor manages quality control to meet contract terms. The QASP is designated as a living document subject to regular review and revision in coordination with the contractor.

The performance objectives include maintaining system operational status at a minimum of 95% annually, conducting preventive maintenance on an approved schedule with 100% compliance, responding to support and repair requests according to specified timeframes, submitting monthly maintenance reports by the 5th of each month, conducting system upgrades meeting manufacturer specifications, and maintaining compliance with VA security and safety protocols. Surveillance methods include random inspections, periodic inspections, customer feedback collection, and documentation review. The Contracting Officer's Representative monitors contractor performance through daily work logs, review meetings, and formal performance evaluations. Any serious or consistent failures to meet performance standards may result in corrective actions ranging from contractor notification to contract termination or non-renewal. Required reports include an Initial Assessment Report within 30 days of contract start, Monthly Maintenance Reports, Detailed Repair and Upgrade Reports within 5 days of task completion, and annual Inventory and System Condition Reports.

Quality Assurance Surveillance Plan (QASP) for Public Address Mass Notification System (PAMNS) and Code Blue Help Point System Maintenance Support Services

1.0 Introduction

This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate the services provided by the contractor in accordance with the Statement of Work (SOW) for the maintenance and support of the Public Address Mass Notification System (PAMNS) and the Code Blue Help Point systems at the VA Salt Lake City Healthcare System (SLCHCS).

This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance.

This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.
