About this file
This is a Performance Relevancy Questionnaire (PRQ) template used to evaluate contractor qualifications for construction services to renovate Sterile Processing Service (SPS) spaces at the Ralph H. Johnson VA Medical Center in Charleston, South Carolina under solicitation 36C24726Q0729. Offerors must submit a minimum of three (3) completed PRQs as part of their technical volume to demonstrate experience on similar projects. Each PRQ must showcase a completed or ongoing construction contract exceeding $2,500,000 in value that involved phased construction in an active patient care or occupied healthcare facility setting, including at least three of the following trades: demolition, earthwork, carpentry, HVAC, electrical, plumbing, communications, or fire suppression. Projects must be no more than five (5) years old from the solicitation issue date, or if ongoing, must be at least 70% complete.
To achieve an ACCEPTABLE rating for Factor 1 (Qualifications—Technical), offerors must demonstrate that both the proposed Project Manager and Onsite Superintendent have a minimum of 5 years of construction supervision experience, and submit at least one PRQ meeting the similarity criteria. The PRQ template requires offerors to provide detailed project information including contract values, completion status, specific work performed, major subcontractors, and relevancy to the scope and complexity of Project 534-24-102. A current point of contact from the owner or government agency must be provided for verification purposes. Offerors certify under penalty that all information is accurate, with misrepresentation potentially resulting in offer rejection and suspension or debarment action.

PERFORMANCE RELEVANCY QUESTIONNAIRE (PRQ)
Project 534-24-102 | Renovate SPS Spaces | Ralph H. Johnson VAMC, Charleston, SC Solicitation No. 36C24726Q0729INSTRUCTIONS
Purpose: This Performance Relevancy Questionnaire (PRQ) is used to evaluate Factor 1 (Qualifications -- Technical) under Part 11.3 of the solicitation. The Government will assess whether the offeror has demonstrated experience on projects similar in scope, complexity, and cost magnitude to Project 534-24-102.
Submission Requirements: Submit one (1) completed PRQ per similar project cited in the Factor 1 qualifications narrative. Offerors must submit a minimum of three (3) PRQs total. PRQs must be submitted as part of Volume I of the offer.
Project Selection Criteria: Select projects that best demonstrate your firm's performance on construction contracts similar to this solicitation. To be considered similar:
(1) SIZE: Project must have a completed or current contract value exceeding $2,500,000.
(2) SCOPE: Project must have involved phased construction in an active patient care or occupied healthcare facility setting, including at minimum three (3) of the following trades: demolition, earthwork, carpentry, HVAC, electrical, plumbing, communications, or fire suppression.
(3) RECENCY: Projects may be ongoing if at least 70% complete. Projects completed more than five (5) years prior to the solicitation issue date will not be considered.
Acceptability Standard: Per Part 11.3 of the solicitation, a rating of ACCEPTABLE for Factor 1 requires the Project Manager and Onsite Superintendent to each have a minimum of 5 years of construction supervision experience AND at least one (1) completed PRQ demonstrating a similar project as defined above. Failure to submit a PRQ meeting the above criteria will result in a rating of UNACCEPTABLE for Factor 1.
