About this file
This document is a Past Performance Questionnaire for a federal contract opportunity related to Event Support Services for the United States Air Force Academy.
The questionnaire requests information from organizations that have previously worked with the offeror, including contract details, performance ratings across various criteria, and an overall performance assessment. The purpose is to verify the offeror's past performance and ability to meet the requirements of the proposed Event Support Services effort. The contracting officer requires a written response to the questionnaire by September 26, 2024 to support the evaluation process. The solicitation for the Event Support Services requirement has been posted to SAM.gov, with a site visit scheduled for September 5, 2024. This is a 100% Small Business 8(a) competitive Set-Aside requirement to provide trained personnel, equipment, materials, and other services for intercollegiate sporting events, parades, and graduations at the US Air Force Academy.

Attachment 5 - Event Support Services Solicitation: Past Performance Questionnaire
Controlled Unclassified Information Page 1 of 6
The 10th Contracting Squadron of the United States Air Force Academy is in the process of competitively selecting a source for a Athletic Department Event Support Services requirement. The Event Support Services requirement is to provide trained personnel, equipment, materials, supervision, and other items and services necessary to provide Event Venue Support to the U.S. Air Force Academy (USAFA) Department of Athletics, as well as for other special events held on USAFA. Event support includes intercollegiate home football games at Falcon Stadium, and other intercollegiate home sporting events held at the Cadet Field House, Cadet Gym, and outdoor sports venues. It also covers support for Parades, I-Day, Special Protocol Events, and Graduation week culminating in Graduation Day.
This service requires supervisory oversight and management of patron access to include patron/bag screening, ticket taking, ushering/crowd control, traffic/coning/signage management, parking management, oversight and access to the Falcon Stadium Press Box, and Falcon Stadium East Club.
One of the considerations in proposal evaluation is the verification of the offerors' past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.
Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 26 Sep 2024. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to kenneth.helgeson.3@us.af.mil and melinda.lingley@us.af.mil. Your completed questionnaire will become a part of the official source selection records.
NOTE: If referenced contract is not a federal contract, some information i.e. Contractor Cage Code, may not be applicable and therefore is not required for completion of the form.
Your assistance is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
