About this file

This is a Past Performance Questionnaire (PPQ) template for federal contract solicitation 140D0426Q0750 for the Historical Preservation Training Center President's House Reinforced Masonry Barrier Wall project.

Section I, completed by the offeror, requires submission of company information (name, address, project manager contact details), referenced contract details (project name, contract number, owner/agency, location, contract type, period of performance, and contract values), a description of similar work with checkboxes for reinforced masonry walls, structural masonry, concrete foundations, site improvements, historic preservation, and work in historically sensitive environments. Offerors must also provide a project description and, if applicable, details on subcontractor or affiliate roles with supporting documentation (letter of commitment or teaming agreement), and identify a client reference with organization name, reference contact name, title, phone, and email.

Section II, completed by the client or project owner, requires performance ratings across 13 evaluation criteria using a six-point scale (Excellent, Very Good, Satisfactory, Marginal, Unsatisfactory, N/A). The evaluation categories are quality of work, schedule performance, cost control, construction management, reinforced masonry construction, structural masonry work, concrete foundations, site improvements, historic preservation/historically sensitive environment work, safety, communication and responsiveness, problem resolution, and overall performance. Each category includes space for detailed comments. The questionnaire concludes with a question on whether the client would award another project to the contractor (Yes, No, or With Reservations) and certification by the client contact affirming familiarity with the project and accuracy of the evaluation.
