About this file
The document is a Past Performance Questionnaire for a USDA Forest Service Request for Quote (RFQ) #12318725Q0171 for Law Enforcement & Investigations (LE&I) Unmarked Vehicle Upfit. The questionnaire is designed to collect detailed performance evaluation information from previous clients or contractors, focusing on three primary assessment areas: quality of product/service, program management/timeliness, and cost control. Contractors are required to submit this form for each referenced project to demonstrate past performance experience.
The solicitation involves upfitting 5 patrol vehicles for USDA Forest Service Region 13 Zone 5, with the vendor responsible for vehicle pickup, delivery, and acquisition of upfitting supplies and services. The evaluation will use a rating scale of Acceptable (low performance risk), Neutral (unverified references), or Unacceptable (historical performance failures). The completed questionnaire must be sent directly to Drackus Thomas, Contract Specialist, via email, with the goal of providing a comprehensive assessment of the contractor's capabilities, performance history, and potential risk factors for the current vehicle upfit project.
