About this file
This is a Past Performance Questionnaire template designed to evaluate contractor performance on relevant past contracts for the Academic Skills Training (AST) program solicitation H9821026RE002. The questionnaire is completed by government evaluators or assessing officials and submitted to the Defense Human Resources Activity (DHRA) points of contact—Joel Auer (joel.d.auer.civ@mail.mil) and Mike Carnley (charles.m.carnley.civ@mail.mil)—by the RFP proposal due date. The form collects general contract information including contractor name, customer agency, contract number, period of performance, contract value, and service description, then requires raters to assess contractor performance across six evaluation categories: Quality of Product or Service, Timeliness of Performance/Schedule, Cost Control, Good Business Relations, Management of Key Personnel, and Regulatory Compliance.
Evaluators rate performance using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) aligned with the Contractor Performance Assessment Reporting System (CPARS), with detailed definitions provided for each rating level. Ratings above or below Satisfactory require written justification supported by specific events and their impact to the government. The questionnaire is designed to provide past performance references that will inform source selection decisions for the Academic Skills Training program, which seeks a commercially available, computer-adaptive, web-based course to improve cognitive, metacognitive, and non-cognitive academic skills for active, reserve, and National Guard Service members across all six military branches.

BACKGROUND
A Request for Proposal (RFP) has been issued to solicit offers that would provide the best value to the Government for the requirements of an acquisition. The specific RFP requirements may be referenced at SAM.gov under H9821026RE002.
Research indicates that students frequently graduate high school with deficits in college and career readiness. This includes gaps in the academic skills of postsecondary students, with approximately half of students needing remediation. Service members, who are a subset of the non-traditional adult student population, are not unique in needing additional academic skills training. To address those needs, DoD sponsors the Academic Skills Training Program in accordance with DoDD 1322.08E, DoDI 1322.26, DoDI 1322.25, and the DoD VolEd Strategic Plan. DoD 1322.08E directs the Department to maintain education programs that are voluntary and comparable to civilian opportunities available to citizens outside of the military.
Specifically, DoDI 1322.25 para. 2.a.4 requires the Department to maintain an "... academic skills program which allows personnel to upgrade their reading, writing, computation, and communication abilities in support of academic skills and military occupations and careers. Academic skills programs may include English as a second language and basic science.
The acquisition of basic educational and academic skills is essential to successful job performance and engaging in new learning experiences. The Department currently maintains two primary methods of Academic Skills Training program delivery: 1) Service-specific face-to-face instruction, 2) virtual instruction, and 2) self-paced, web based courseware.
Department offers a commercial, web-based academic skills product equally accessible to all Service members that encompasses basic through pre- college level math and English skills. In FY24, there were more than 40,000 Service members enrolled as new users in academic skills training.
The Department has the capacity to deliver Academic Skills Training (AST) that:
a. Enables flexibility across the Services to adapt the program to meet Service members' needs
b. Expands access to multiple instructional strategies and delivery methods to address various learning styles and the increased support required for remediation

c. Incorporates collaboration as a support to reduce attrition among high-needs learners (i.e. instructors, peers, etc.). The U.S. Department of Education defines high-needs learners are defined as young people 18 to 25 at risk of educational failure or with difficulties learning and who require additional support to help users progress and achieve.
d. Provides additional cognitive, metacognitive (i.e. self-awareness, the ability to self-assess, etc.), and non-cognitive (i.e. time management, self-discipline, self-advocacy, etc.) skills development
e. Integrates AST as a support throughout Service members' military lifecycle
f. Leverages quality resources to support learning
g. Monitors, evaluates, and documents Service member skills improvement related to measurable program outcomes (i.e. Armed Services Vocational Aptitude Battery (ASVAB) general technical (GT) and Armed Forces Qualification Test (AFQT) composite score improvement, college-level course placement, etc.)

QUESTIONNAIRE INSTRUCTIONS
This questionnaire covers the topics that are in line with the Contractor Performance Assessment Reporting System (CPARS). This questionnaire is designed to provide basic information regarding the Contractor’s present/past performance results on the contracts they have been awarded with your company and your satisfaction with their performance.
Please use the following guide in preparing your response to the performance assessments:
Exceptional Definition: Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Note: To justify an Exceptional rating, identify multiple significant events and state how they were of benefit to the Government. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO significant weaknesses identified.
Very Good Definition: Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Note: To justify a Very Good rating, identify a significant event and state how it was a benefit to the Government. There should have been no significant weaknesses identified.
Satisfactory Definition: Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Note: To justify a Satisfactory rating, there should have been only minor problems, or major problems the contractor recovered from without impact to the contract/order. There should have been NO significant weaknesses identified. A fundamental principle of assigning ratings is that contractors will not be evaluated with a rating lower than Satisfactory solely for not performing beyond the requirements of the contract/order.
Marginal Definition: Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.
Note: To justify Marginal performance, identify a significant event in each category that the contractor had trouble overcoming and state how it impacted the Government. A Marginal rating should be supported by referencing the management tool that notified the contractor of the contractual deficiency (e.g., management, quality, safety, or environmental deficiency report or letter).
Unsatisfactory Definition: Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
Note: To justify an Unsatisfactory rating, identify multiple significant events in each category that the contractor had trouble overcoming and state how it impacted the Government. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating. An Unsatisfactory rating should be supported by referencing the management tools used to notify the contractor of the contractual deficiencies (e.g., management, quality, safety, or environmental deficiency reports, or letters).
