About this file

This document is a Past Performance Questionnaire issued by the Federal Bureau of Investigation (FBI) Finance and Facilities Division, Procurement Section, dated October 18, 2023, for a Target Vehicles acquisition. The questionnaire is designed to evaluate a contractor's past performance across six key criteria: Quality of Service, Schedule, Cost Control, Business Relations, Management of Key Personnel, and Utilization of Small Business.

The document provides a detailed rating framework with five performance levels for each criterion: Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory. The purpose is to obtain feedback from previous contract references about a contractor's performance. Upon completion, the form should be emailed directly to the Contracting Officer Javia Warner and Contract Specialist Treyton Miller, with a specific subject line format (15F06726Q0000042 – Target Vehicles – Past Performance Questionnaire – [Company Name]). The final section includes an overall assessment question asking whether the evaluator would have reservations about soliciting the contractor for future critical and demanding programs.
