GenAI Reporting Form and Contract Summary

This document is a Generative Artificial Intelligence (GenAI) Reporting Form (CPUC 1000A, Rev. 02/2025) issued by the California Public Utilities Commission (CPUC) in connection with RFQ 24ITS0885 for Advice Letter Website Updates and Maintenance. The form requires bidders and contractors to disclose any use of GenAI technology, models, services, or systems proposed to meet the contract's requirements. Contractors must complete the form if they intend to use or offer GenAI products and if those products will process State of California data. The form requests identification of GenAI business processes, data classifications, vendor information, product specifications including model versions and license tiers, delivery methods (IaaS, PaaS, SaaS, or on-premises), workflow overviews, proposed functionality, and security safeguards. Contractors must also identify any disabled GenAI features and specify whether tailored training and rules of engagement will be required for proper system use.

The CPUC CIO or designee must complete a separate GenAI Risk Assessment (SIMM 5305-F) and conduct a California Department of Technology (CDT) consultation to determine the risk level of proposed GenAI use, categorized as Low Risk, Moderate Risk (requiring CDT consultation prior to award), or High Risk (requiring further CDT consultation prior to award). Failure to provide requested information may result in bidder disqualification or contract voidance. The form includes specific instructions for each section, definitions of key terms, and designated sections for CPUC use only. All contractors must sign the form and commit to submitting updated CPUC 1000A forms if any new or previously unreported GenAI use is identified during contract performance.
