Addendum to FAR 52.212-1 – Instructions to Offerors – Commercial items.

Quote Preparation Instructions-General

Quote is expected to conform to the solicitation provisions and be prepared in accordance with this section. To aid in evaluation, the quote shall be clearly and concisely written as well as being neat, indexed (crossed-indexed as appropriate) and logically assembled. All pages of each part shall be appropriately numbered and identified with the name of the offeror, the date and the solicitation number.

Delivery

Offeror shall deliver quote via electronic mail to the above stated point of contacts.

Overall Arrangement of Quote

The overall quote shall consist of two (2) separate volumes/documents, individually entitled as stated below:

Volume I Offer and Other Documents Volume II Price Quote

Volume I – Offer and Other Documents

This volume shall contain the offeror’s contractual documentation. These documents will be reviewed only for compliance with solicitation, FAR, or other statutory requirements and will not be evaluated as part of the source selection factors identified in the Evaluation Section.

This volume consists of the actual offeror to enter into a contract to perform the desired work. It also includes required representations, certifications, and acknowledgements, small business subcontracting plan, justification for noncompetitive proposed subcontracts, identification of technical data to be withheld, any other administrative information, and a summary of exceptions and deviations taken. Although it incorporates them by reference, it does not physically include the other volumes.

Format and Content

Volume I - Offer and Other Documents, shall include the following documents (in the order listed):

a. The quote form fully executed (signed). The Standard Form 1449 is to be executed fully (to include amendments when applicable) and used as the cover sheet (or first page) of Volume I - Offer and Other Documents. The person signing the quote form must have the authority to commit the offeror to all of the provisions of the quote, fully mailto:lhariper@blm.gov mailto:chgonzalez@blm.gov

Yuma Field Office Vault Pumping Services recognizing that the Government has the right, by terms of the solicitation, to make an award without further discussion if it so elects.

b. Offeror Representations, Certifications, and Other Statements fully executed as required pursuant FAR 52.212-3 – Offeror Representations and Certifications – Commercial Items. This provision is in the System for Acquisition Management (SAM.gov) where offerors should ensure the information is up-to-date. If this information is in SAM.gov then the offeror is not required to provide with their quote. If offeror does not have an active SAM Registration then print FAR Clause 52.212-3, complete, and provided with the quote submission.

c. Small Business Subcontracting Plan in accordance with FAR Clause 52.219-9 if subcontracting possibilities exist.

d. Exceptions and Deviations taken to the contract and/or solicitation requirement and summary of Exceptions and Deviations taken on other Volumes.

Volume II: Price Quote

The price quote shall provide firm fixed unit prices as identified in Attachment 3 – Bid Schedule. Firm fixed unit pricing is required for all performance periods (each year/contract line-item numbers (CLINs)), then multiple unit price by the estimated quantity for a subtotal, then total these subtotals for a total period of performance (CLIN) value, and then total each period of performance for a total quoted value. Offeror’s quoted unit prices shall include, but are not limited to, all labor, equipment, materials, State and Federal taxes including workman’s compensation.

QUOTE SUBMISSION CHECKLIST

a. Completed Blocks 17a, 17b (if applicable), 30a, 30b and 30c of the Standard Form 1449.

b. Acknowledged receipt of all solicitation amendments (if applicable).

c. Representations and Certifications: Ensure Representations and Certifications are up to date in the System for Award Management (SAM). If offeror does not have an active SAM Registration, then print FAR Clause 52.212-3, complete, and provided with the quote submission. Additionally, if offeror does not have an active SAM Registration, they must print FAR 52.209-7, Information Regarding Responsibility Matters and complete.

d. Provided documents as required in Attachment 1 – Statement of Work (SOW).

e. Read entire FAR Clause 52.212-1, Instructions to Offerors-Commercial and Addendum to ensure compliance.

Questions:

Questions must be submitted to the Contract Specialist and Contracting Officer via email. The deadline to submit questions is July 7, 2022, by 2:00 pm (PS).

Quotes:

Quotes must be submitted to the Contract Specialist and Contracting Officer via email. The deadline to submit quotes is July 14, 2022, by 2:00 pm (PS).
