The questionnaire is designed to evaluate contractor past performance and will be used as part of the proposal evaluation process.

The questionnaire contains two sections. Section A, completed by the offeror (prospective contractor), collects general contract information including the offeror's business address, point of contact, phone number, assessor contract award information, description of services provided, contract award date, contract amount, and period of performance or delivery date. The offeror must provide the complete document to each assessor and submit only Section A copies with their proposal by the solicitation closing date. Section B, completed by independent assessors, evaluates contractor performance across three categories: Quality of Service (contractual compliance and quality of goods/services), Schedule (on-time delivery and invoicing accuracy/timeliness), and Business Relations (business practices, working relationships, reporting and invoice timeliness, and complaint resolution responsiveness). Assessors rate each criterion as either Acceptable or Unacceptable, with Unacceptable ratings requiring written explanation. Assessors must submit completed questionnaires electronically to amy.tilley@usda.gov no later than September 4, 2026 by 12:00 noon Eastern Time, and must not share assessment information with the offeror being evaluated.
