About this file
This document appears to be an attachment providing service level agreement (SLA) options for a contract opportunity. The vendor is proposing SLA terms to ensure the availability and operational status of their equipment, specifically Multi-function Devices (MDFs). The key SLA options include providing a loaner MDF if the primary MDF cannot be made operational within a certain number of days, reducing rental fees if the MDF does not meet minimum uptime percentages for a month or quarter, and replacing failed MDFs with new units. The vendor also proposes providing additional loaner MDFs if a certain percentage of on-site units are non-operational.
The document does not provide details on the overall contract opportunity, such as the agencies or jurisdictions involved, the products or services being procured, response dates, award information, or contract terms. Pricing terms, funding sources, and other salient commercial details are also not included in this attachment.

Attachment 5 SLA Options
As part of our proposal, ________(Vendor Name)______ will provide the following service level agreements:
___ If a MDF cannot be made operational within ____ days, a comparable loaner MDF will be made available until the MDF is operational again.
___ If a MDF fails to meet the minimum uptime of ____ % for an entire month, then ____ % of the machine rental will be reduced from the bill.
___ If a MDF fails to meet the minimum uptime of ____ % for an entire quarter, the vendor will replace the MDF with a NEW machine of equal or better specifications and features.
___ If more than ____ % of the medium and large capacity MDFs at one site are down, the vendor will provide a loaner MDF until some of the machines are restored to service.
Press list any exceptions or exclusions that are part of the SLA agreement.
Vendors may also propose a unique offer of SLA options to ensure that MDF machines are not out of service for extended periods of time which is disruptive to operations.
