About this file
This document is a Representations and Certifications template that offerors must complete and submit as part of their response to a federal solicitation if they have not already completed annual representations electronically in the System for Award Management (SAM).
The document contains five key Federal Acquisition Regulation (FAR) and Defense FAR Supplement (DFARS) provisions. FAR 52.223-22 requires offerors who received $7.5 million or more in federal contract awards in the prior fiscal year to disclose whether they publicly report greenhouse gas emissions and quantitative emissions reduction goals, with instructions to provide relevant website URLs if applicable. FAR 52.204-24 addresses telecommunications and video surveillance equipment or services, requiring offerors to disclose whether they will provide covered telecommunications equipment or services to the government and whether they use such equipment internally, with detailed disclosure requirements including equipment descriptions, entity information, brand/model numbers, and explanations of proposed use relative to applicable prohibitions. FAR 52.204-26 contains similar but simplified representations regarding covered telecommunications equipment or services. DFARS 252.204-7016 requires representation regarding covered defense telecommunications equipment or services, with mandatory SAM review of excluded parties. DFARS 252.204-7017 imposes a prohibition on acquiring covered defense telecommunications equipment or services for covered missions and requires detailed disclosures including equipment descriptions, service provider information, CAGE codes, OEM or distributor status, and explanations of proposed use relative to the applicable prohibition. Collectively, these provisions establish compliance requirements for offerors regarding environmental disclosures and telecommunications security restrictions.
